米索前列醇联合双氯芬酸钠在阴道超声引导下绝经后节育环嵌顿取环术中的应用

摘　　要：目的探讨米索前列醇联合双氯芬酸钠在阴道超声引导下绝经后节育环嵌顿取环术中的临床应用效果。方法将52例绝经后宫内节育器(IUD)嵌顿取环妇女随机分成观察组27例和对照组25例,观察组术前口服米索前列醇片、肛门塞双氯芬酸钠栓,在阴道超声引导下取环;对照组仅口服米索前列醇后行常规取环术。比较2组取出成功率、疼痛程度、手术时间及出血量。结果观察组取环成功26例(96.30%),失败1例(3.70%);1例(3.74%)感重度疼痛,17例(62.96%)感轻度疼痛;手术时间为(5.37±2.01)min;术中出血量为(5.73±2.48)mL。对照组取环成功19例(76.00%),失败6例(24.00%);10例(40.00%)感重度疼痛,1例(4.00%)感轻度疼痛;手术时间为(10.35±3.01)min,术中出血量为(9.52±3.06)mL。2组取出成功率、疼痛程度、手术时间及出血量比较差异均有统计学意义(P<0.05或P<0.001)。结论米索前列醇联合双氯芬酸钠在阴道超声指引下绝经后节育环嵌顿取出术可缩短手术时间,止痛效果显著,术中出血量减少,手术成功率高,且阴道超声引导可及时发现宫腔内异常情况。Objective To explore the clinical application of misoprostol combined with diclofenac sodium in transvaginal ultrasound-guided removal of incarcerated intrauterine device(IUD)in postmenopausal women.Methods Fifty-two postmenopausal women undergoing removal of incarcerated IUD were divided into two groups.The observation group(n=27)was given transvaginal ultrasound-guided removal of IUD after preoperative treatment with oral misoprostol tablets and rectal diclofenac sodium suppository.The control group(n=25)only received routine IUD removal after oral misoprostol administration.The removal success rate,pain degree,operation time and blood loss were compared between the two groups.Results In the observation group,IUD removal was successful in 26 cases(96.30%),and failed in 1 case(3.70%).In the control group,IUD removal was successful in 19 cases(76.00%),and failed in 6 cases(24.00%).The incidence of severe pain,incidence of mild pain,operation time and intraoperative blood loss were,respectively,3.74%(1 case),62.96%(17 cases),(5.37±2.01)min and(5.73±2.48)mL in the observation group,and 40.00%(10 cases),4.00%(1 case),(10.35±3.01)min and(9.52±3.06)mL in the control group.There were significant differences in the removal success rate,pain degree,operation time and blood loss between the two groups(P<0.05 or P<0.001).Conclusion The application of misoprostol combined with diclofenac sodium in transvaginal ultrasound-guided removal of IUD can shorten operation time,relieve pain,reduce intraoperative blood loss and improve operation success rate in postmenopausal women.In addition,transvaginal ultrasound guidance can timely detect the abnormal conditions in the uterine cavity.
