摘　　要：目的探讨咽鼓管圆枕形态与胆脂瘤型中耳炎乳突根治术后不干耳的关系。方法选取胆脂瘤型中耳炎患者60例(60耳),均行乳突根治术治疗。术前行电子鼻咽镜检查,观察鼻咽部咽鼓管圆枕形态;术中采用咽鼓管探查法评估咽鼓管功能。比较圆枕分叶咽鼓管通畅患者(A组)、圆枕分叶咽鼓管欠通畅患者(B组)及圆枕不分叶患者(C组)术后6个月不干耳发生情况。结果术后6个月A组、B组、C组不干耳发生率分别为60.0%、57.1%、13.9%,C组不干耳发生率显著低于A组、B组,差异均有统计学意义(χ^(2)=6.32、9.80,P=0.0125、0.0023)。结论咽鼓管圆枕形态与胆脂瘤型中耳炎术后不干耳发生有关,术前应重视患者的鼻咽部状态检查及评估咽鼓管功能,以便指导临床围手术期治疗和术后预防不干耳的发生。Objective To investigate the correlation between torus tubarius shape and otorrhea after radical mastoidectomy for cholesteatomatous otitis media.Methods Sixty patients with cholesteatomatous otitis media(60 ears)underwent radical mastoidectomy.The shape of torus tubarius was observed by electronic nasopharyngoscopy before surgery,and eustachian tube function was assessed using exploration method during operation.At 6 months after operation,the incidence of otorrhea was compared among patients with lobulated torus tubarius eustachian tube patency(group A),patients with lobulated torus tubarius eustachian tube obstruction(group B),and patients with non-lobulated torus tubarius(group C).Results The incidence of otorrhea in group C(13.9%)was lower than that in group A(60.0%)and group B(57.1%)(χ^(2)=6.32 and 9.80,respectively;P=0.0125 and 0.0023,respectively).Conclusion The shape of torus tubarius is related to otorrhea after radical mastoidectomy for cholesteatomatous otitis media.In order to guide perioperative treatment and prevent the occurrence of otorrhea,attention should be paid to preoperative examination of nasopharyngeal status and assessment of eustachian tube function.
