自体富血小板血浆联合角膜绷带镜治疗轻中度眼热烧伤的临床观察

摘　　要：目的:观察自体富血小板血浆(PRP)联合角膜绷带镜治疗轻中度眼热烧伤的疗效。方法:选取2019年6月—2021年9月湖南省浏阳市人民医院收治的63例(86眼)I~II度眼热烧伤患者为研究对象,随机分为两组。PRP组33例患者共45患眼,其中I度烧伤14眼,II度烧伤31眼。对照组30例患者共41患眼,其中I度烧伤12眼,II度烧伤29眼。对照组采用抗感染、营养支持等全身治疗以及局部用药的方式治疗,PRP组在此基础上予以角膜绷带镜覆盖后加用自体PRP外用滴眼,共治疗8周。比较两组患者角膜愈合时间,以及随访4周和8周时视力恢复情况、眼干燥症发生率、角膜透明度变化和新生血管发生率。结果:PRP组眼角膜愈合时间为(5.6±2.4)d,对照组为(8.9±3.1)d,PRP组愈合时间明显缩短(P<0.05)。随访4周时PRP组与对照组分别有18眼(40.0%)和30眼(73.2%)发生眼干燥症,随访8周时PRP组与对照组分别有20眼(44.4%)和31眼(75.6%)出现眼干燥症,两个时间点PRP组眼干燥症发生率均明显少于对照组(P<0.05);两组患者在视力恢复程度、角膜感染发生率、角膜透明度变化、角膜新生血管发生率等方面差异均无统计学意义(P>0.05)。结论:自体PRP联合角膜绷带镜治疗轻中度眼热烧伤可明显缩短角膜修复时间,降低伤后眼干燥症发生率,同时未增加角膜感染率,是治疗轻中度眼热烧伤的安全有效的治疗方法。Objective:To observe the efficacy of autologous platelet-rich plasma(PRP)combined with corneal bandage lens in the treatment of mild to moderate thermal eye burns.Methods:A total of 63 patients(86 eyes)with I-II degree thermal eye burns admitted to the People’s Hospital of Liuyang from Jun.2019 to Sept.2021 were selected as the research subjects and randomly divided into two groups.In the PRP group,there were 33 patients with 45 eyes,including 14 eyes with first-degree burns and 31 eyes with second-degree burns.In the control group,there were 30patients with 41 eyes,including 12 eyes with first-degree burns and 29 eyes with second-degree burns.Patients in the control group received systemic therapy such as anti-infection,nutritional support and local medication.Patients in the PRP group received corneal bandage lens covering and external eye drops of autologous platelet-rich plasma for 8weeks.The healing time was observed and compared between the two groups,as well as the visual recovery,incidence of xerophthalmia,changes in corneal transparency and incidence of neovascularization at 4 and 8 weeks of follow-up.Results:The corneal healing time of the PRP group was 5.6±2.4 days,and that of the control group was 8.9±3.1 days.The healing time of the PRP group was significantly shortened(P<0.05).After a 4-week follow-up,18 eyes(40.0%)in the PRP group and 30 eyes(73.2%)in the control group developed xerophthalmia.After 8 weeks of follow-up,20eyes(44.4%)in the PRP group and 31 eyes(75.6%)in the control group developed xerophthalmia.The incidence of xerophthalmia in the PRP group was significantly lower than that of control group at both time points(P<0.05).There were no significant differences in the degree of visual recovery,incidence of corneal infection,change of corneal transparency and incidence of corneal neovascularization between the two groups(P>0.05).Conclusions:Autologous platelet-rich plasma combined with corneal bandage lens can significantly shorten the corneal healing time of thermal eye burns,reduce
