甲氧西林金黄色葡萄球菌的葡萄球菌盒式染色体基因分型及耐药特性分析

摘　　要：目的探讨耐甲氧西林金黄色葡萄球菌(MRSA)SCCmec基因分型及耐药特性,为临床有效治疗不同感染类型的菌株提供理论支持。方法收集2019年1—12月本院住院患者检出的MRSA96株进行16种抗菌药物耐药性检测,采用聚合酶链反应(PCR)技术对MRSA的mecA及SCCmec基因进行检测。结果96株MRSA对万古霉素、利奈唑胺、喹努普汀/达福普汀、替加环素敏感率为100%,对青霉素G、苯唑西林、头孢西丁耐药率为100%,对克林霉素、红霉素、庆大霉素、左氧氟沙星、莫西沙星、利福平、复方新诺明、四环素、环丙沙星耐药率分别为76.04%、77.08%、8.33%、22.92%、14.58%、6.25%、6.25%、37.5%、27.08%。96株MRSA中mecA基因阳性为86株(89.58%),SCCmec基因分型结果显示:SCCmecⅡ型16株(16.67%)、SCCmecⅢ型25株(26.04%)、SCCmecⅣa型51株(53.13%)、SCCmecⅣb型1株(1.04%),未分型3株(3.12%)、未检出SCCmecⅠ型、SCCmecⅣc型、SCCmecⅣd型、SCCmecⅤ型。结论我院分离的MRSA菌株以SCCmecⅣa型为主,临床治疗MRSA感染时应根据细菌药敏结果合理选用抗菌药物。Objective To discuss the SCCmec gene typing and drug resistance characteristics of methicillin-resistant Staphylococcus aureus(MRSA),and to provide theoretical support for clinically treating patients infected with different type of bacterial strain effectively.Methods A total of 96 strains of MRSA were collected from inpatients of Wanbei Coal and Electricity Group General Hospital from January to December,2019.Drug resistance of 16 kinds of antibiotics were detected using the 96 strains of MRSA.The mecA gene and SCCmec genotype of MRSA were detected by polymerase chain reaction(PCR).Results A 100 percent of the 96 MRSA strains were sensitive to vancomycin,linezolid,quinoprotein/dafurtin,tigecycline.The drug resistance to penicillin G,oxacillin,cefoxitin were 100%,and their drug resistance to clindamycin,erythromycin,gentamicin,levofloxacin,moxifloxacin,rifampicin,cotrimoxazole,tetracycline,and ciprofloxacin were 76.04%,77.08%,8.33%,22.92%,14.58%,6.25%,6.25%,37.5%,and 27.08%,respectively.Among the 96 MRSA strains,86 strains(89.58%)were positive for mecA gene.The results of SCCmec genotyping showed that there were 16 strains(16.67%)of SCCmec typeⅡ,25 strains(26.04%)of SCCmec typeⅢ,51 strains(53.13%)of SCCmecⅣa,1 strain(1.04%)of SCCmecⅣb,3 strains(3.12%)were not typed,and no SCCmecⅠ,SCCmecⅣc,SCCmecⅣd and SCCmecⅤwere detected.Conclusions The main MRSA strains isolated in our hospital are SCCmecⅣa,clinicians should choose antibiotics reasonably according to the results of drug sensitivity.
