摘  要:目的描述和分析1990—2019年中国高体质指数(高BMI)归因疾病负担状况及变化趋势,为我国高BMI科学防控提供依据。方法提取2019年全球疾病负担数据中中国高BMI归因死亡、伤残调整寿命年(DALYs)、早死损失寿命年(YLLs)和伤残损失寿命年(YLDs)等疾病负担指标,采用年度变化百分比(APC)和平均年度变化百分比(AAPC)分析率的变化趋势,采用Excel软件进行统计分析。结果30年来,高BMI归因死亡数从23.5万人增至76.47万人,DALYs从787.63万人年增至2483万人年,YLLs从651.34万人年增至1831.91万人年,YLDs从136.29万人年增至651.09万人年(标化率分别增长36.07%、44.43%、28.19%和125.05%)。经标化后,死亡率、DALYs率、YLLs率和YLDs率整体呈上升趋势(AAPC分别为1.0%,1.3%、0.9%和2.8%,均P<0.05)。亚组分析发现,男女性别比范围除YLDs率外均大于1;高BMI归因疾病负担随年龄增长显著增加。结论近30年我国BMI疾病负担日趋沉重,应持续加强高BMI的筛查和健康干预,对男性、高龄人等重点人群采取针对性措施,以减小健康损失和提高生存质量。Objective To describe and analyze the high body mass index(high BMI)attributable disease burden status and changing trends in China from 1990 to 2019,and provide a basis for scientific prevention and control of high BMI in China.Methods The disease burden indicators of death,disability-adjusted life years(DALYs),premature death years of life lost(YLLs)and years lived with disability(YLDs)attributable to high BMI in China from the 2019 global burden of disease data were extracted,and the trends of rate changes were analyzed using annual percentage change(APC)and average annual percentage change(AAPC),and Excel software was used for statistical analysis.Results Over 30 years,the number of deaths attributable to high BMI increased from 235000 to 764700,DALYs from 7876300 to 24830000 person-years,YLLs from6513400 to 18319100 person-years,and YLDs from 1362900 to 6510900 person-years(the standardized rates increased by 36.07%,44.43%,28.19%,and 125.05%).After standardization,there was an overall increasing trend in mortality,DALYs rate,YLLs rate,and YLDs rate(AAPC:1.0%,1.3%,0.9%,and 2.8%,respectively,all P<0.05).Subgroup analysis revealed that the range of male to female sex ratio was greater than 1 except for the YLDs rate;the burden of disease attributed to high BMI increased significantly with age.Conclusion The burden of BMI disease in China has become increasingly heavy in the last 30 years,and screening and health interventions for high BMI should be continuously strengthened.Targeted measures should be taken for key populations such as men and senior citizens to reduce health loss and improve survival quality.
