摘  要:目的:观察大鼠胃窦Cajal间质细胞(interstitial cells of cajal,ICC)自噬蛋白表达与凋亡,探讨电针改善糖尿病胃轻瘫(diabetic gastroparesis,DGP)大鼠胃动力的可能作用机制。方法:64只SD大鼠随机分为对照组和造模组,采用腹腔注射链脲佐菌素(streptozocin,STZ)及高脂高糖饲料不规则喂养制备DGP大鼠模型,8周后将造模成功大鼠随机分为模型组、电针组、胃复安组,另设对照组各每组10只。电针组选取“梁门”(ST21)“足三里”(ST36)“三阴交”(SP6)穴行电针治疗,并用0.9%氯化钠溶液灌胃(1 mL/100 g),胃复安组用1.7%胃复安药液灌胃(1 mL/100 g),模型组和对照组均用0.9%氯化钠溶液灌胃(1 mL/100 g),治疗前后测量大鼠血糖值,酚红灌胃法计算胃排空率,提取胃窦ICC原代细胞,采用Western blot法检测自噬蛋白微管相关蛋白1轻链3(microtubule-associated protein 1 light chain 3,LC3)、家蚕隔离体蛋白1(sequestosome-1,p62)表达,流式细胞仪检测ICC细胞凋亡。结果:与对照组比较,模型组血糖值、自噬蛋白微管相关蛋白1轻链3-Ⅱ/微管相关蛋白1轻链3-Ⅰ(LC3-Ⅱ/Ⅰ)比值、P62相对表达量、ICC细胞凋亡率均显著升高(P<0.01),胃排空率显著下降(P<0.01)。与模型组比较,电针组血糖值下降(P<0.05),LC3-Ⅱ/Ⅰ比值、P62相对表达量、ICC细胞凋亡率均显著下降(P<0.01),胃排空率显著升高(P<0.01)。结论:电针能提高DGP大鼠胃动力,改善大鼠血糖值,其机制可能与促进胃窦ICC细胞自噬并降低其凋亡率、调节自噬与凋亡的平衡有关。Objective:To study the mechanism of electroacupuncture improving gastric motility in diabetic gastroparesis(DGP)rats by observing autophagy protein expression and apoptosis of interstitial cells of Cajal(ICC)in gastric antrum.Methods:64 sprague-dawley(SD)rats were randomly divided into control and model groups,the animal model of DGP was established by intraperitoneal injection of streptozotocin(STZ)and high glucose and high fat diet.After 8 weeks,the successfully modeled rats were randomly divided into 3 groups:model group,electroacupuncture group,metoclopramide group,with 10 rats in each group.The control group was also set up with 10 rats.Electroacupuncture group was given electroacupuncture treatment on ST21,ST36 and SP6,and 0.9%NaCl solution intragastrically(1 mL/100 g).Metoclopramide group was given 1.7%metoclopramide solution intragastrically(1 mL/100 g).Model group and control group were given 0.9%NaCl solution intragastrically(1 mL/100 g).The blood glucose level was measured before and after treatment,gastric emptying rate was calculated by phenol red gavage,ICC primary cell was extracted from gastric antrum,the expression of LC3 protein and P62 protein was measured by Western blot.Flow cytometry was used to analyze the apoptosis of ICCs.Results:Compared with the control group,blood glucose and LC3Ⅱ/Ⅰ,the expression of P62,ICCs apoptosis rate were increased significantly in the model group(P<0.01),gastric emptying rate was decreased significantly(P<0.01).Compared with model group,blood glucose was decreased(P<0.05),LC3Ⅱ/Ⅰ,the expression of P62,and ICCs apoptosis rate were decreased significantly in electroacupuncture group(P<0.01),gastric emptying rate was increased significantly(P<0.01).Conclusion:Electroacupuncture can improve the gastric motility and ameliorate the blood glucose level of DGP rats,its mechanism may be related to the promotion of autophagy of gastric antrum ICCs and the reduction of their apoptosis rate,and the regulation of the balance between autophagy and apoptosis.
