基于名老中医经验的肺痿病证候辨证规则量化研究

摘　　要：目的:挖掘名老中医对肺痿病的辨证经验并量化展示,以期更好地继承发扬名老中医的学术经验。方法:基于已建立的《现代名老中医肺病数据库》和《期刊中医肺病数据库》,获取名老中医对肺痿进行辨证的症状与证候数据共412例,涉及88个症状25种证候,采用统计分析、改进贝叶斯统计进行分析挖掘。结果:对症状、证候进行统计分析得到符合条件的63个症状和16种证候,使用改进贝叶斯统计求出16个证候阈值、相应症状分值,得到定量化辨证经验规则。以表虚证规则为例,其阈值为10,症状分值从高至低依次为自汗10、神疲5.89、头痛4.77、恶寒3.4、乏力2.46、纳呆2.34、盗汗2.17等。对所有规则进行测试,其辨证准确率最高98.54%,最低87.38%,平均94.49%。结论:挖掘名老中医对肺痿的辨证经验基本符合中医理论,能够量化地反映数据中症状和证候的内在关系。Objective:In order to better inherit and carry forward the academic experience of famous veteran doctors of traditional Chinese medicine,the author excavated the experience of syndrome differentiation rules of pulmonary flaccidity syndrome and presented it quantitatively.Method:Based on the established databases of Modern Famous TCM Lung Disease,and Academic Journals TCM Lung Disease,the datas of 412 patients with pulmonary flaccidity syndrome,involving 88 symptoms and 25 syndromes,were obtained.Statistical analysis and improved Bayesian statistics were used for analysis and mining data.Results:After statistical analysis,63 symptoms and 16 syndromes were obtained.16 syndrome thresholds and corresponding symptom scores were obtained by using improved Bayesian statistics,and the quantitative syndrome differentiation rules were obtained.Such as the rule of superficial deficiency syndrome,the threshold value was 10,and the symptom scores value were sweating 10,mental fatigue 5.89,headache 4.77,aversion to cold 3.4,lacking in strength 2.46,anorexia 2.34,night sweats 2.17,etc.All the rules were tested and verified,the highest syndrome classification accuracy was 98.54%,the lowest was 87.38%and the average was 94.49%.Conclusion:The experience of syndrome differentiation rules of pulmonary flaccidity syndrome from famous veteran doctors was basically accord with the theory of Chinese medicine.The results can quantitatively reflect the potential relationship between symptoms and syndromes.
