自拟养血化瘀汤治疗缺血性脑卒中后吞咽障碍的疗效观察

摘　　要：目的 探讨自拟养血化瘀汤治疗缺血性脑卒中后吞咽障碍的临床疗效。方法 选取湖州市第一人民医院2019年4月~2020年10月治疗的100例缺血性脑卒中后吞咽障碍患者,按照随机奇偶数字表法分为两组,对照组50例采用常规治疗,观察组50例在对照组基础上给予自拟养血化瘀汤治疗,对比两组的临床疗效、神经功能、血液流变学、吞咽功能、不良反应发生率、生活质量。结果 观察组总有效率高于对照组(P<0.05);治疗前,两组的神经功能及吞咽功能评分比较,差异无统计学意义(均P>0.05);治疗后,观察组评分低于对照组(均P<0.05);治疗前,两组的全血黏度高切、全血黏度低切等血液流变学指标比较,差异无统计学意义(均P>0.05);治疗后,观察组的全血黏度高切、全血黏度低切低于对照组(均P<0.05);两组的不良反应发生情况比较,差异无统计学意义(P>0.05);治疗前,生活质量各项评分比较,差异无统计学意义(P>0.05);治疗后,观察组的生活质量各项评分均低于对照组(P<0.05)。结论 自拟养血化瘀汤治疗缺血性脑卒可改善血液流变学指标,提高临床疗效。Objective To explore the clinical efficacy of self-made Yangxue Huayu Decoction in the treatment of swallowing dysphagia after ischemic stroke. Methods A total of 100 patients with post-ischemic stroke dysphagia treated in our hospital from Apirl 2019 to October 2020 were divided into two groups according to the random odd and even number method. The control group 50 cases received conventional treatment,and the observation group 50 cases were in the control group. On the basis of the group,self-made Yangxue Huayu Decoction was given, and the clinical efficacy,neurological function, hemorheology, swallowing function,incidence of adverse reactions, and quality of life were compared between the two groups. Results The total effective rate of the observation group was higher than that of the control group(P<0.05);Before treatment, the scores of nerve function and swallowing function were compared between the two groups(all P>0.05);after treatment, the scores of the observation group were lower than the control group(all P<0.05).Comparison of hemorheological indicators such as high blood viscosity and low blood viscosity between the two groups before treatment(all P>0.05);after treatment, the above indicators in the observation group were lower than the control group(all P<0.05).The two groups were poor Comparison of the occurrence of reactions(P>0.05).Comparison of various scores of quality of life before treatment(P>0.05);After treatment, the quality of life scores of the observation group were lower than the control group(P<0.05). Conclusion Self-made Yangxue Huayu Decoction can improve blood rheology indexes and improve clinical efficacy in the treatment of ischemic stroke.
