止咳平喘十二味合剂治疗痰热郁肺型咳嗽变异性哮喘的临床疗效评价及作用机制研究

摘　　要：目的 探讨止咳平喘十二味合剂在痰热郁肺型咳嗽变异性哮喘中的应用效果,总结该治疗方法的作用机制。方法 选取2017年9月~2018年8月宁波市中医院呼吸内科门诊就诊的痰热郁肺型咳嗽变异性哮喘患者100例,随机分为对照组(常规西药治疗)和治疗组(止咳平喘十二味合剂),每组各50例。观察两组的咳嗽症状评分、咳嗽生活质量评分,观察两组肺功能情况,两组血清相关因子变化,分别于治疗前、治疗后检测定患者血清,记录血清总IgE、血嗜酸性粒细胞(EOS)百分比、血清白介素8(IL-8)等。结果 观察组的咳嗽症状总分低于对照组,差异有统计学意义(P<0.05);观察组的咳嗽生活质量评分低于对照组,差异有统计学意义(P<0.05);观察组的肺功能高于对照组,差异有统计学意义(P<0.05);观察组的血清因子低于对照组,差异有统计学意义(P<0.05)。结论 止咳平喘十二味合剂可有效减轻痰热郁肺型咳嗽变异性哮喘的咳嗽症状,提高咳嗽生活质量,改善肺功能,对血清炎症因子有抑制性作用,临床应用效果显著,可推广于临床。Objective To observe the application effect of Zhike Pingchuan Shierwei mistura in cough variant asthma(CVA) with syndrome of phlegm-heat obstructing lung, and to summarize the mechanism of this treatment method.Methods A total of 100 CVA patients with syndrome of phlegm-heat obstructing lung who attended the outpatient clinic of the department of respiratory medicine in our hospital from September 2017 to August 2018 were selected and randomly divided into the control group(n=50, treated with conventional western medicine) and the treatment group(n=50,treated with Zhike Pingchuan Shierwei mistura). Results The total cough symptom score of the observation group was lower than that of the control group, with statistically significant differences(P<0.05).The cough-specific quality of life scale scores were lower in the observation group than those in the control group,with statistically significant differences(P<0.05).The lung function of the observation group was better than that of the control group,with statistically significant differences(P<0.05).The levels of serum factors in the observation group were lower than those in the control group, with statistically significant differences(P<0.05). Conclusion Zhike Pingchuan Shierwei mistura can effectively alleviate cough symptoms, enhance the quality of life during cough and improve lung function for CVA patients with syndrome of phlegm-heat obstructing lung, and has an inhibitory effect on serum inflammatory factors. It has remarkable application effects and is worthy of promotion in clinic.
