揿针联合中风复元方治疗脑卒中后吞咽功能障碍的疗效及对血清IGF-1、BDNF、VEGF水平的影响

摘　　要：目的 探讨揿针联合中风复元方治疗脑卒中后吞咽功能障碍的疗效及对血清胰岛素样生长因子(IGF-1)、血管内皮生长因子(VEGF)、脑源性神经营养因子(BDNF)水平的影响。方法 选取浙江省人民医院2017年9月~2021年1月收治的124例脑卒中患者,采用随机数字表法分为试验组(n=62)和对照组(n=62),两组均给予常规基础治疗,对照组在此基础上给予中风复元方治疗,试验组在此基础上给予揿针联合中风复元方治疗,连续治疗2个疗程,对比两组患者的治疗总有效率;对比两组患者治疗前后功能性经口摄食量表(FOIS)评分、才藤分级、标准吞咽功能评定法(SSA)评分及神经营养指标(IGF-1、BDNF、VEGF);对比两组患者住院时间、吞咽功能平均恢复时间。结果 试验组总有效率高于对照组(P<0.05);治疗后,试验组FOIS评分和才藤分级高于对照组,SSA评分低于对照组(P<0.05);治疗后,低于对照组试验组血清IGF-1、BDNF和VEGF水平高于对照组(P<0.05);试验组住院时间和吞咽功能平均恢复时间短于对照组(P<0.05)。结论 揿针联合中风复元方治疗脑卒中后吞咽功能障碍的疗效较高,可有效提高患者吞咽功能和血清IGF-1、BDNF、VEGF水平。Objective To explore the effect of thumbtack needle combined with Zhongfeng Fuyuan Decoration in the treatment of dysphagia after stroke and its influence on serum insulin-like growth factor(IGF-1), vascular endothelial growth factor(VEGF) and brain-derived neurotrophic factor(BDNF) levels. Methods A total of 124 stroke patients admitted to Zhejiang Provincial People′s Hospital from September 2017 to January 2021 were selected. They were divided into the experimental group(n=62) and the control group(n=62) using random number table method. Both groups were given conventional basic treatment. The control group were given Zhongfeng Fuyuanfang Decoration, and the experimental group were given thumbtack needle treatment combined with Zhongfeng Fuyuanfang Decoration. The treatment was performed for 2 consecutive courses. The total effective rate was compared between the two groups. The functional oral intake scale(FOIS) scores, dysphagia severity scale(DSS) scores, standard swallowing assessment(SSA) scores, and neurotrophic indicators(IGF-1, BDNF, VEGF) before and after treatment were compared between the two groups. The length of hospital stay and average time of swallowing function recovery were compared between the two groups. Results The total effective rate in the experimental group was significantly higher than that in the control group(P<0.05).After treatment, compared with the control group, the experimental group had significantly higher FOIS and DSS scores and significantly lower SSA score(P<0.05). After treatment, compared with the control group, the experimental group had significantly higher serum IGF-1, BDNF and VEGF levels(P<0.05). The length of hospital stay and average time of swallowing function recovery in the experimental group were significantly shorter than those in the control group(P<0.05). Conclusion Thumbtack needle combined with Zhongfeng Fuyuan Decoction is effective in the treatment of dysphagia after stroke. It can effectively improve swallowing function and serum IGF-1, BDNF and VEGF
