尿微量蛋白、臂-踝脉搏波传导速度与老年隐蔽性高血压的相关性研究

摘　　要：目的 探讨尿微量蛋白(microalbuminuria,MA)、臂-踝脉搏波传导速度(brachial-ankle pulse wave velocity,baPWV)与老年隐蔽性高血压(masked hypertension,MH)的关系。方法 在参加2010至2011年度健康体检的开滦离退休员工中整群抽取年龄≥60岁的人群2814例于2012至2014年进行动态血压监测,符合入选标准者2464例,分析其中448例诊室血压正常老年人MA、baPWV与MH的关系。结果 448例观察对象患者中MH 188例。其中,单纯白天MH 10例(5.3%),单纯夜间MH 119例(63.3%),白天和夜间MH 59例(31.4%)。与正常血压组相比,白天和夜间MH组的MA升高(P<0.05);单纯白天MH、单纯夜间MH、白天和夜间MH组baPWV增快(P<0.05)。回归结果显示,MA>30 mg/L与总人群MH和男性MH正相关,OR值分别是2.016(95%CI 1.076~3.779)、2.886(95%CI 1.271~6.554),与女性MH无关。baPWV≥1554 cm/s与总人群MH、男性MH、女性MH正相关,OR值分别为3.249(95%CI 2.084~5.064)、4.163(2.340~7.406)、2.378(95%CI 1.197~4.727)。结论 老年单纯白天MH、单纯夜间MH、白天和夜间MH比例分别为5.3%、63.3%和31.4%,尿微量蛋白>30 mg/L、baPWV>1554 cm/s与老年MH正相关。Objective To investigate the correlation between microalbuminuria,brachial-ankle pulse wave velocity(baPWV)and msked hypertension(MH)in elderly patients.Methods A total of 2814 retired staff aged over 60 years who participated in 2010~2011 physical examination were selected by cluster sampling for ambulated blood pressure monitoring during 2012~2014,and the eligible subjects were 2,464.Moreover,the correlation between microalbuminuria(MA),baPWV and MH was analyzed in 448 elderly subjects with normal blood pressure.Results Of the 448 participants with MH,there were 10 cases of simple daytime MH(dMH),accounting for 5.3%,and 119 cases of simple night-time MH(nMH),accounting for 63.3%,and 59 cases of day-night MH(dnMH),accounting for 31.4%.Compared with those in normotensive group,the MA levels in dnMH group were significantly increased(P<0.05),and the baPWV levels in dMH grous,nMH group and dnMH group were significantly increased(P<0.05).Logistic regression result showed:MA over 30mg/L was positively related to total and male MH.however,which was not related to female MH.And BaPWV>1,554cm/s was positively related to total MH,male and female MH,with OR values being 3.249(95%CI 2.084~5.064),4.163(2.340~7.406),2.378(95%CI 1.197~4.727),respectively.Conclusion The proportions of dMH,nMH,dnMH in elderly subjects are 5.3%,63.3%and 31.4%,respectively.MA over 30mg/L and baPWV>1,554cm/s are positively related to MH in the elderly.
