摘  要:目的 研究神经生长因子(nerve growth factor,NGF)和前列腺素F2α受体(prostaglandin F2αreceptor,PTGFR)在子宫腺肌病(adenomyosis)患者异位内膜组织中的表达情况,探讨其与子宫腺肌病痛经的关系。方法 选取妇科2017年1月至2019年1月因子宫腺肌病行全子宫或次全子宫切除术患者40例为试验组,以同期子宫肌瘤患者40例为对照组(行全子宫或次全子宫切除术)。免疫组化检测子宫腺肌病患者异位内膜组织、子宫肌瘤患者内膜组织中NGF、PTGFR的表达强度。术前评估患者痛经程度(采用视觉模拟评分法visual analogue scale/score,VAS),子宫腺肌病组:轻度痛经(0~3分)14例,中度痛经(4~6分)16例,重度痛经(7~10分)10例。对照组:均为轻度痛经。结果 子宫腺肌病异位内膜组织中NGF、PTGFR表达均高于对照组,差异有统计学意义(P<0.05)。子宫腺肌病异位内膜组织中NGF、PTGFR的表达水平随痛经程度的加重而增加(P<0.05)。结论 子宫腺肌病患者异位内膜组织中NGF及PTGFR表达水平增高,且其表达水平随痛经加重而增高,两者在子宫腺肌病痛经发生中可能存在协调作用。Objective To investigate the expressions of nerve growth factor(NGF)and prostaglandin F2αreceptor(PTGFR)in heterotopic endometrial tissue of patients with adenomyosis(AM),and to explore the correlation between expression of NGF,PTGFR and dysmenorrhea due to adenomyosis.Methods Forty patients with AM who underwent total or subtotal hysterectomy in our hospital from January 2016 to January 2018 were enrolled as experimental group,at the same time,the other 40 patients with hysteromyoma who underwent total or subtotal hysterectomy due wee enrolled as control group.The expression levels of NGF and PTGFR in heterotopic endometrium tissues of patients with AM and in eutopic endometrum tissues of patients with hysteromyoma detected by immunohistochemistry.The dysmenorrhea degree of patients was evaluated by visual analogue scale/score(VAS)before surgery.In AM group,there were 14 cases of mild dysmenorrhea(0~3 points),16 cases of moderate dysmenorrhea(4~6 points),and 10 cases of severe dysmenorrhea(7~10 points).All the cases in control group were of mild dysmenorrhea(0~3 points).Results The expression levels of NGF and PTGFR in heterotopic endometrium tissues of patients with AM were significantly higher than those in control group(P<0.05).The expression levels NGF and PTGFR in heterotopic endometrium of patients with AM were increased with the aggravation of dysmenorrhea degree.Conclusion The expression levels NGF and PTGFR in heterotopic endometrium of patients with AM are increased with the aggravation of dysmenorrhea degree,and the two factors may have a coordination effect in the occurrence of AM.
