宫颈残端癌预防相关研究进展

摘　　要：宫颈残端癌临床上较少见,但近年来,女性对健康及生活质量要求不断提高,要求保留宫颈的患者增多,子宫次全切除术的数量也逐渐增多,以致宫颈残端癌发生的潜在风险增加。且其治疗较一般宫颈癌难度较大、并发症较多,预后较差。因此,宫颈残端癌的预防需引起临床医生的重视。宫颈残端癌的发病率同行子宫次全切除术的数量有关,在子宫次全切除术前需严格掌握手术指征,术后规范随访。了解其致病因素,如宫颈HPV感染、不良性生活史、流产次数与孕产史、宫颈慢性炎症、阴道微生态环境的变化、外源性雌激素、吸烟和被动吸烟、子宫全切术后性防护等,预防宫颈残端癌的发生。The cervical stump cancer is relatively rare in clinical practice,but in recent years,women’s requirements for health and life quality are increasing constantly,the number of patients requiring cervical preservation is increasing,and the number of subtotal hysterectomy is also gradually increasing,so that the potential risk of cervical stump cancer is increasing.At present,there is no unified standard for the treatment of cervical stump cancer,which is more difficult,more complications and worse prognosis than general cervical cancer.Therefore,the prevention of the disease needs to be paid much attention by clinicians.The incidence rate of cervical stump cancer is related to the number of subtotal hysterectomy.The indication of operation should be strictly controlled before subtotal hysterectomy and follow up is standard after operation.Therefor it is necessary to understand the pathogenic factors,such as cervical HPV infection,adverse sexual life history,abortion frequency and pregnancy history,cervical chronic inflammation,changes of vaginal micro ecological environment,exogenous estrogen,smoking and passive smoking,sexual protection after total hysterectomy and so on,so as to prevent the occurrence of cervical stump cancer.
