摘 要:目的:探讨双层探测器光谱计算机体层成像(Computed Tomography,CT)智能剂量调控技术在低剂量胸部体检中的应用价值。方法:选取2021年8月~2022年2月在中山大学附属第三医院行双层探测器光谱CT低剂量胸部体检的成人60例,年龄35~65岁,平均(51.6±10.2)岁,随机分为智能剂量调控技术扫描组(观察组)和常规固定50mAs管电流扫描组(对照组),每组各30例。比较两组图像在肺尖、中肺、下肺三个层面的SD值、信噪比SNR以及比较两组图像的容积CT剂量指数CTDIvol值。2名医师独立双盲采用5分法评价观察组和对照组肺尖、中肺、下肺的图像质量。结果:观察组肺尖的SD值低于对照组,差异具有统计学意义(P<0.05);两组图像中肺、下肺的SD值比较,差异无统计学意义(P>0.05)。观察组肺尖的SNR值高于对照组,差异具有统计学意义(P<0.05);两组图像中肺、下肺的SNR值比较,差异无统计学意义(P>0.05)。观察组体重指数≥27kg/m2时CTDIvol值高于对照组,差异具有统计学意义(P<0.05);两组图像体重指数<27kg/m2时CTDIvol值比较,差异无统计学意义(P>0.05)。观察组肺尖的主观评分高于对照组,差异具有统计学意义(P<0.05);两组图像中肺、下肺的主观评分比较,差异无统计学意义(P>0.05)。结论:在对健康体检者应用双层探测器光谱CT智能剂量调控技术行低剂量胸部扫描时,可以对个体差异以及胸部不同部位进行智能调控,从而保证了胸部整体的图像质量。Objective:To explore the application value of spectral CT intelligent dose control with double detector in low dose chest physical examination.Methods:From August 2021 to February 2022,60 adults,aged 35-65 years,with an average of(51.6±10.2)years old,who underwent low-dose chest physical examination with double-layer detector spectral CT in the Third Affiliated Hospital of Sun Yat-Sen University,were randomly selected.They were randomly divided into intelligent dose control scanning group(study group)and conventional fixed 50mAs tube current scanning group(control group),with 30 cases in each group.SD values and SNR of the two groups of images at apex,middle lung and lower lung levels,as well as CTDIvol values of the two groups of images were compared.Two physicians independently and double-blind used the 5-point method to evaluate the image quality of the lung apex,middle lung and lower lung in the observation group and the control group.Results:The SD value of lung apex in the study group was lower than that in the control group,and the difference was statistically significant(P<0.05).SD values of lung and lower lung in the two groups showed no significant difference(P>0.05).The SNR value of lung tip in the study group was higher than that in the control group,and the difference was statistically significant(P<0.05).There was no significant difference in the SNR values of lung and lower lung between the two groups(P>0.05).CTDIvol value in the study group was higher than that in the control group when BMI≥27kg/m2(P<0.05).There was no significant difference in CTDIvol value between the two groups when BMI<27kg/m2(P>0.05).The subjective score of lung apex in study group was higher than that in control group,and the difference was statistically significant(P<0.05).There was no significant difference in subjective scores of lung and lower lung between the two groups(P>0.05).Conclusion:In the low-dose chest scan of healthy people using the intelligent dose regulation technology of spectral CT with double detector,
