摘 要:目的:调查研究产前超声检查用于胎儿肾积水诊断中的效果。方法:从2012年10月~2015年5月本院收治的产前使用超声检查肾积水患儿中随机抽取200例作为研究对象,通过对胎儿出生前后肾积水症状进行追踪观察分析。结果:产前超声诊断等级共分为三级,等级越低积水程度越轻。经过追踪调查,产前超声检测肾盂前后径为6~9mm的胎儿最终都未因肾积水引起病理性尿路疾病;产前超声检测肾盂前后径为10~15mm的胎儿大部分都未因肾积水引起病理性尿路疾病,只有7.14%的患儿在之后有出现尿路梗阻现象。产前超声检测肾盂前后径在15mm以上的胎儿其中接近一半的患儿在出生后被检测出病理性的尿路梗阻。胎儿肾积水发病初发期和高峰期中主要集中在一、二级;肾积水初发期的输尿管内径、羊水指数和肾盂APD与高峰期比较有统计学意义(P<0.05)。结论:产前超声检查用于胎儿肾积水诊断中的效果明显,能有效的协助医师进行准确的临床诊断。可以通过直观的二维图像对胎儿在腹中的情况进行预判。Objective:To investigate the effect of prenatal ultrasound in the diagnosis of fetal hydronephrosis.Methods:From October 2012 to May 2015 in our hospital,200 children with hydronephrosis undergoing prenatal ultrasound examination were randomly selected as the research objects,and the fetal hydronephrosis symptoms before and after birth were followed up and analyzed.Results:There are three levels of prenatal ultrasound diagnosis.The lower the level,the lighter the degree of water accumulation.After follow-up investigation,the fetuses whose anterior and posterior renal pelvis diameter was 6-9mm by prenatal ultrasound did not eventually cause pathological urinary tract disease;Most of the fetuses whose anterior and posterior renal pelvis diameter was 10-15mm were not caused by kidney.Water accumulation causes pathological urinary tract disease,and only 7.14%of children have urinary tract obstruction afterwards.Prenatal ultrasound detection of fetuses with a renal pelvis diameter of 15mm or more,and nearly half of the children were detected with pathological urinary tract obstruction after birth.The incidence of fetal hydronephrosis was mainly concentrated in the first and second grade in the initial stage and peak period;The ureteral diameter,amniotic fluid index and renal pelvis APD in the initial stage of hydronephrosis were significantly higher than those in the peak stage(P<0.05).Conclusion:Prenatal ultrasound is effective in the diagnosis of fetal hydronephrosis,and it can effectively assist physicians in accurate clinical diagnosis.It is possible to predict the condition of the fetus in the abdomen through intuitive two-dimensional images.
