摘 要:目的 研究彩色多普勒超声(彩超)鉴别诊断乳腺肿块良恶性的价值。方法 120例乳腺肿块患者,经手术病理检查证实良性病变100例、恶性病变20例。患者均行彩超诊断,以病理诊断结果为金标准,分析彩超诊断结果 ,比较良恶性乳腺肿块患者的影像学特征及血流信号分级。结果 病理诊断显示,良性病变患者100例、恶性病变患者20例。彩超诊断显示,良性病变检出率为97.00%(97/100)、恶性病变检出率为90.00%(18/20)。恶性病变患者的包膜不完整、形态不规则、内部钙化、边界毛刺、腋窝淋巴结异常占比分别为75.00%、95.00%、55.00%、90.00%、80.00%,均高于良性病变患者的8.00%、4.00%、3.00%、3.00%、2.00%,差异具有统计学意义(P<0.05)。恶性病变患者的血流信号分级差于良性病变患者,差异具有统计学意义(Z=7.280, P<0.05)。结论 彩超检查乳腺肿块具有较高的检出率,另外,可以有效鉴别诊断良恶性病变在超声图像特征、血流特点方面的差异,有助于早期治疗。Objective To study the value of color Doppler ultrasound in the differential diagnosis of benign and malignant breast masses. Methods Among the 120 patients with breast masses, 100 cases of benign lesions and 20 cases of malignant lesions were confirmed by surgical and pathological examination. All patients underwent color Doppler ultrasound. Taking pathological findings as the gold standard, the results of color Doppler ultrasound were analyzed, and the imaging features and blood flow signal grading of patients with benign and malignant breast masses were compared. Results Pathological diagnosis showed that there were 100 patients with benign lesions and 20 patients with malignant lesions. Color Doppler ultrasound showed that the detection rate of benign lesions was 97.00%(97/100), and the detection rate of malignant lesions was 90.00%(18/20). The proportions of incomplete capsule, irregular shape, internal calcification, border burrs, and abnormal axillary lymph nodes in patients with malignant lesions were 75.00%, 95.00%, 55.00%, 90.00%, and 80.00%, which were higher than 8.00%, 4.00%, 3.00%, 3.00%, and 2.00% of patients with benign lesions, and the differences were statistically significant(P<0.05). The blood flow signal grading of malignant lesions was worse than that of benign lesions, and the difference was statistically significant(Z=7.280, P<0.05). Conclusion Color Doppler ultrasound has a high accuracy rate in detecting breast masses. In addition, it can effectively differentiate and diagnose the differences in ultrasound image characteristics and blood flow characteristics of benign and malignant lesions,which is helpful for early treatment.
