小剂量米非司酮联合戊酸雌二醇治疗围绝经期功能性子宫出血的疗效观察

摘　　要：目的 评价小剂量米非司酮联合戊酸雌二醇治疗围绝经期功能性子宫出血(FUB)的临床效果。方法 86例围绝经期FUB患者,按照随机法分为对照组和观察组,每组43例。对照组患者采用戊酸雌二醇治疗,观察组患者在对照组基础上配合小剂量米非司酮口服治疗。比较两组患者临床疗效及相关指标[住院时间、子宫停止出血时间、血红蛋白、子宫内膜厚度以及卵泡刺激素(FSH)、黄体生成素(LH)、雌二醇(E_(2))水平。]。结果 治疗后,观察组患者的治疗总有效率100.00%高于对照组的74.42%,差异具有统计学意义(P<0.05)。治疗后,观察组患者的血红蛋白(102.50±3.50)g/L、FSH(36.98±3.20)IU/L、LH(19.65±2.05)IU/L、E_(2)(20.60±2.05)ng/L均低于对照组的(122.50±8.80)g/L、(41.50±3.50)IU/L、(26.20±2.50)IU/L、(25.50±2.50)ng/L,子宫内膜厚度(6.68±0.85)mm小于对照组的(8.65±1.01)mm,子宫停止出血时间(22.50±1.50)d和住院时间(8.50±1.50)d均短于对照组的(36.50±2.20)、(10.50±1.50)d,差异具有统计学意义(P<0.05)。结论 小剂量米非司酮联合戊酸雌二醇治疗围绝经期FUB效果理想,可有效降低了患者的子宫内膜厚度、性激素水平,加速患者康复。Objective To evaluate the clinical effect of low-dose mifepristone combined with estradiol valerate in the treatment of perimenopausal functional uterine bleeding (FUB).Methods A total of 86 patients with perimenopausal FUB were randomly divided into control group and observation group,with 43 cases in each group.The control group was treated with estradiol valerate,and the observation group was treated with low-dose mifepristone on the basis of the control group.Both groups were compared in terms of clinical efficacy,related indicators[hospitalization time,time to cessation of uterine bleeding,hemoglobin,endometrial thickness,folliclestimulating hormone (FSH),luteinizing hormone (LH),and estradiol (E_(2))].Results After treatment,the total effective rate of treatment 100.00%of the observation group was higher than 74.42%of the control group,and the difference was statistically significant (P<0.05).After treatment,the hemoglobin (102.50±3.50) g/L,FSH(36.98±3.20) IU/L,LH (19.65±2.05) IU/L and E_(2)(20.60±2.05) ng/L of the observation group were lower than(122.50±8.80) g/L,(41.50±3.50) IU/L,(26.20±2.50) IU/L and (25.50±2.50) ng/L of the control group;the endometrial thickness (6.68±0.85) mm was less than (8.65±1.01) mm of the control group;the time to cessation of uterine bleeding (22.50±1.50) d and hospitalization time (8.50±1.50) d were shorter than (36.50±2.20) and(10.50±1.50) d of the control group;all the differences were statistically significant (P<0.05).Conclusion Lowdose mifepristone combined with estradiol valerate has an ideal effect in the treatment of perimenopausal FUB,which can effectively reduce the endometrial thickness and the level of sex hormones,and accelerate the recovery of the patient.
