摘  要:目的探讨小剂量叶酸片治疗H型高血压的效果及其MTHFR C677T基因多态性的关系。方法回顾性分析178例H型高血压患者的临床资料,检测H型高血压患者MTHFR C677T基因型,依据基因检测结果将患者分为CC、CT、TT 3组。比较3组患者叶酸片治疗前后叶酸、同型半胱氨酸(Hcy)水平。结果治疗前TT型患者血浆Hcy最高,比较CT、CC组,差异有统计学意义(P<0.01)。H型高血压经叶酸治疗后1个月,TT型Hcy水平较CT、CC组明显降低,差异有统计学意义(P<0.05)。对于3种基因型,治疗1个月,Hcy水平低于治疗前,治疗3个月后3组Hcy水平低于治疗1个月后,差异均有统计学意义(P<0.05)。治疗3个月后CC、CT、TT 3组间叶酸水平变化差值无显著性差异(P>0.05)。结论H型高血压患者MTHFR C677T基因TT型Hcy水平最高,短期应用小剂量叶酸可以降低H型高血压Hcy水平,尤其是TT型,应及早干预。Objective To investigate the effect of low-dose folic acid tablets in the treatment of H-type hypertension and the relationship between its MTHFR C677T gene polymorphism.Methods The clinical data of 178 patients with H-type hypertension were retrospectively analyzed,and the MTHFR C677T genotype of patients with H-type hypertension was detected,and the patients were divided into three groups,CC,CT and TT,based on the genetic test results.Compare three groups of patients with folic acid tablets before and after folic acid and same homocysteine(Hcy).Results Plasma Hcy was highest in TT patients before treatment,and the difference was statistically significant when comparing the CT and CC groups(P<0.01).1 month after treatment with folic acid for H hypertension,Hcy levels were significantly lower in the TT group compared with the CT and CC groups,and the difference was statistically significant(P<0.05).For the three genotypes,the levels of Hcy were lower at 1 month of treatment and before treatment,and the levels of Hcy in the three groups were lower after 3 months of treatment than after 1 month of treatment,and the differences were statistically significant(P<0.05).There was no significant difference in the difference of folate level changes among CC,CT and TT groups after 3 months of treatment(P>0.05).Conclusion Hcy levels are highest in patients with MTHFR C677T gene TT type in H-type hypertension.Short-term application of low-dose folic acid can reduce Hcy levels in H-type hypertension,especially in TT type,and should be intervened early.
