摘　　要：目的分析原发性高血压患者静息心率与颈动脉斑块、血浆同型半胱氨酸、胱抑素C的相关性。方法选择108例原发性高血压患者根据静息心率分为快心率组(静息心率≥80次/min,n=47)和非快心率组(静息心率<80次/min,n=61)。所有患者均行颈部血管彩超,获得颈动脉斑块Crouse积分,抽取空腹血获得血浆同型半胱氨酸、胱抑素C等指标。比较两组颈动脉斑块Crouse积分、血浆同型半胱氨酸、胱抑素C等。结果①快心率组年龄及高血压病程明显大于非快心率组(P<0.05)。②快心率组颈动脉斑块Crouse积分、内膜增厚检出率较非快心率组明显升高(P<0.01)。③快心率组血浆同型半胱氨酸、胱抑素C较非快心率组明显升高(P<0.05)。④静息心率与颈动脉斑块Crouse积分、年龄、高血压病程、血浆同型半胱氨酸成正相关。⑤颈动脉斑块Crouse积分与静息心率、年龄、高血压病程、血浆同型半胱氨酸、胱抑素C成正相关。结论静息心率越快年龄越大、高血压病程越长、颈动脉损害越严重、血浆同型半胱氨酸及胱抑素C越高。Objective To analysis the relationships of resting heart rate and carotid artery plaque,plasma homocysteine,cystatin C in primary hypertension patients.Methods Choose 108 cases patients with essential hypertension and they were divided into fast heart rate group(resting heart rate≥80 times/min,n=47)and non-fast heart rate group(resting heart rate<80 times/min,n=61)according to the resting heart rate.All patients underwent cervical color Doppler ultrasound to obtain carotid plaque Crouse score,and fasting blood was drawn to obtain plasma homocysteine,cystatin C and other indicators.Compare the differences between the two groups of Crouse plaque score,plasma homocysteine,cystatin C etc.Results(1)The age and course of hypertension of fast heart rate group was obviously higher than that of the non-fast heart rate group(P<0.05).(2)As compared to non-fast heart rate group,the detection rate of intimal thickening and Crouse score of plaque in fast heart group were significantly increased(P<0.01).(3)As compared to non-fast heart rate group,plasma homocysteine and cystatin C in fast heart group were significantly increased(P<0.05).(4)Resting heart rate was positively correlated with Crouse plaque score,and with age,and with duration of hypertension,and with plasma homocysteine.(5)The Crouse plaque score was positively correlated with resting heart rate,and with age,and with duration of hypertension,and with plasma homocysteine,and with cystatin C.Conclusions The faster the resting heart rate and the older the age,the longer duration of hypertension,the more serious the carotid artery damage,the higher the plasma homocysteine and cystatin C.
