血清前蛋白转化酶枯草溶菌素9在2型糖尿病合并冠心病患者中的表达分析

摘　　要：目的探讨血清前蛋白转化酶枯草溶菌素9在2型糖尿病合并冠心病患者中的表达及其临床意义分析。方法回顾性分析2019年12月至2021年8月在我院治疗的54例2型糖尿病合并冠心病患者的临床资料,将其作为A组,将同期54例单纯2型糖尿病患者作为B组,将同期54例健康体检者作为C组,对比3组的血糖、血脂等各血生化指标,分析血清前蛋白转化酶枯草溶菌素9(PCSK9)与各指标的相关性。结果A组PCSK9、FPG、HbA1c、FINS、TC、TG、LDL-C、hs-CRP、Hcy、IL-6水平均明显高于B组、C组,HDL-C水平明显低于B组、C组,差异有统计学意义(P<0.05);Pearson分析显示,2型糖尿病合并冠心病患者PCSK9水平与FINS、TC、TG、LDL-C、hs-CRP、Hcy、IL-6呈正相关(r=0.269,0.215,0.207,0.304,0.241,0.273,0.256,P<0.05),与FPG、HbA1c、HDL-C无明显相关性(r=0.128,0.117,-0.102,P>0.05)。结论PCSK9在2型糖尿病合并冠心病患者中呈高表达,与FINS、TC、TG、LDL-C、hs-CRP、Hcy、IL-6呈正相关,可影响脂质代谢,加重炎性反应,还能降解载脂蛋白E受体2和肝细胞表面的LDL-C受体,使LDL-C水平上升,影响TC、TG水平,在T2DM合并冠心病患者的诊断与治疗中具有重要价值。Objective Exploring the expression of pre-serum transforme enzyme-proofing fungus 9 in patients with type 2 diabetes and their clinical significance analysis.Methods Retrospective analysis the clinical data of 54 cases of type 2 diabetes combined with coronary heart disease in our hospital from December 2019 to August 2021,they are used as a group A,In the same period,54 patients with type 2 diabetes were used as a group B,and 54 patients with health checkups were used as group C.Compared with the blood glucose,blood lipids and other blood biochemical indicators in 3 groups,analyze the correlation between pre-serum protein conversion enzymes and deodorine 9(PCSK9)and each indicator.Results Group A PCSK9,FPG,HbA1c,FINS,TC,TG,LDL-C,HS-CRP,HCY,IL-6 are significantly higher than that the difference was statistically significant(P<0.05).Pearson analysis shows that PCSK9 levels of patients with type 2 diabetes with coronary heart disease are positively correlated with FINS,TC,TG,LDL-C,HS-CRP,HCY,IL-6(r=0.269,0.215,0.207,0.304,0.241,0.273,0.256,P<0.05),no obvious correlation with FPG,HbA1c,HDL-C(r=0.128,0.117,-0.102,P>0.05).Conclusion PCSK9 is highly expressed in patients with type 2 diabetes combined with coronary heart disease,which is positively related to FINS,TC,TG,LDL-C,HS-CRP,HCY,IL-6 The LDL-C receptor on the surface of Lidal 2 and the surface of the hepatocytes is increased to increase the level of LDL-C,which affects TC and TG levels.It has important value in the diagnosis and treatment of patients with coronary heart disease in T2DM.
