老年AECOPD患者应用匀浆膳治疗的疗效及对免疫、营养状况的影响

摘　　要：目的探讨老年慢性阻塞性肺疾病急性加重(AECOPD)患者应用匀浆膳治疗的疗效及对免疫、营养状况的影响。方法纳入2018年1月~2020年10月浙江中医药大学附属杭州市中医院收治的AECOPD患者70例,用随机数字表法均分两组,每组各35例。对照组予以家庭日常饮食干预,观察组给予匀浆膳口服。7 d为1个疗程,连续治疗4个疗程。记录比较住院、吸氧时间、治疗前后营养指标、免疫指标变化及不良反应发生情况。结果治疗后,观察组的住院、吸氧时间均短于对照组(均P<0.05);治疗后,两组的血清总蛋白、白蛋白、前白蛋白、转铁蛋白均升高,且观察组高于对照组(均P<0.05);治疗后,两组的免疫功能均得到改善,观察组的效果优于对照组(均P<0.05);观察组的不良反应发生率为2.86%,低于对照组的20.00%(P<0.05)。结论老年AECOPD患者予以匀浆膳干预可改善患者的免疫功能及营养状况,安全性高,建议推广运用。Objective To investigate the effect of homogenate diet on immune and nutritional status in elderly patients with acute exacerbation of chronic obstructive pulmonary disease(AECOPD).Methods A total of 70 patients with AECOPD treated in Hangzhou Hospital of Traditional Chinese Medicine Affiliated to Zhejiang University of Traditional Chinese Medicine from January 2018 to October 2020 were included.They were randomly divided into two groups with 35 cases in each group.The control group was given family diet intervention,and the observation group was given homogenate diet orally.7 days was a course of treatment,with 4 consecutive courses of treatment.The hospitalization and oxygen inhalation time were compared,and the changes of nutritional indexes,immune indexes and adverse reactions before and after treatment were recorded.Results After treatment,the hospitalization and oxygen inhalation time of the observation group were shorter than those of the control group(all P<0.05);after treatment,the serum total protein,albumin,prealbumin and transferrin of the two groups were increased,and the observation group was higher than the control group(all P<0.05);after treatment,the immune function of the two groups was improved,and the effect of the observation group was better than that of the control group(all P<0.05);the incidence of adverse reactions in the observation group was 2.86%,which was lower than 20.00%in the control group(P<0.05).Conclusion The intervention of homogenate diet in elderly AECOPD patients can improve the immune function and nutritional status of patients with high safety,which was recommended to promote the use of homogenate diet.
