摘  要:目的探究TCD联合弥散张量成像对缺血性脑卒中的诊断价值分析。方法选取2020年3月~2021年3月浙江省温岭市第一人民医院收治的疑似缺血性脑卒中患者100例,病理检查结果为金标准,最终确诊55例为观察组,45例为对照组。通过酶联免疫试验法检测环磷酸腺苷(cAMP)、环磷酸鸟苷(cGMP)水平,分析TCD联合弥散张量成像对缺血性脑卒中的诊断价值。结果相比对照组,观察组的收缩期峰值流速(PSV)、舒张末期血流速度(EDV)、平均血流速度(MV)、各向异性分数(FA)、相对FA(rFA)、表观弥散系数(ADC)值较低,搏动指数(PI)值较高;相比轻、中度患者,重度患者的PSV、EDV、MV、FA、rFA、ADC、cAMP水平较低,PI、cGMP水平较高(均P<0.05)。PSV、EDV、MV、FA、rFA、cAMP水平与病情严重程度呈负相关,PI、cGMP水平与病情严重程度呈正相关(P<0.05)。相比弥散张量成像、TCD,联合诊断对该疾病的诊断效能较高。结论TCD联合弥散张量成像诊断可更为准确地判断缺血性脑卒中的病情发展,对病情严重程度评估具有较高诊断价值。Objective To explore the diagnostic value of TCD combined with diffusion tensor imaging in ischemic stroke.Methods Selected 100 patients with suspected ischemic stroke admitted to the First People's Hospital of Wenling in Zhejiang Province from March 2020 to March 2021,and the pathological examination results were the gold standard,55 cases were confirmed as observation group and 45 cases were in the control group.The levels of adenosine cyclic phosphate(cAMP)and guanosine cyclic phosphate(cGMP)were measured by an enzyme-linked immunoassay,to analyze the value of TCD combined with diffusion tensor imaging in the diagnosis of ischemic stroke.Results Low peak systolic flow velocity(PSV),end diastolic flow velocity(EDV),mean velocity(MV),average flow score(FA),relative FA(rFA),apparent dispersion coefficient(ADC)values,higher beat index(PDI),PSV,EDV,MV,FA,rFA,ADC and cAMP levels and higher PI and c GMP levels(all P<0.05).PSV,EDV,MV,FA,rFA,and cAMP levels were negatively correlated with disease severity,and PI and cGMP levels were positively correlated with disease severity(all P<0.05).Compared with diffusion tensor imaging and TCD,the combined diagnosis is more effective to diagnose the disease.Conclusion The diagnosis of TCD combined with diffusion tensor imaging can more accurately judge the development of ischemic stroke,and has a high diagnostic value for the assessment of disease severity.
