摘 要:目的探讨高血压左室肥厚伴左心力衰竭患者行心脏超声检查的临床价值。方法选取2018年3月~2020年3月于江西省九江市第一人民医院接受治疗的35例高血压左室肥厚伴左心力衰竭患者作为研究组,同时选取同期35例健康体检者作为对照组。两组均接受心脏超声检查,观察研究组、对照组,研究组组内不同美国纽约心脏病协会(NYHA)分级患者的心脏超声诊断结果。结果研究组舒张瓣口最大血流速度/舒张瓣环速度(E/EA值)为(13.54±2.08),左房内径(LAD)为(31.51±4.36)mm,左室舒张末期内径(LVEDD)为(51.58±4.67)mm,左室射血分数(LVEF)为(50.09±3.24)%;研究组E/EA值、LVEDD,均高于对照组,且研究组LAD与LVEF低于对照组(P<0.05)。NYHA分组Ⅲ级组E/A值为(15.99±1.54),LAD为(53.90±1.52)mm,LVEDD为(59.87±5.34)mm,LVEF为(47.24±3.02)%,研究组NYHA分级Ⅲ级患者E/A值、LAD、LVEDD均高于NYHA分级Ⅰ级、Ⅱ级患者,且LVEF均低于NYHA分级Ⅰ级、Ⅱ级患者(P<0.05)。结论在高血压左室肥厚伴左心力衰竭患者病情诊断中进行心脏超声检查,可通过检查情况判断患者心脏病变情况,为下一步病情的诊断与治疗方案的制定提供指导。Objective To investigate the clinical value of echocardiography in patients with hypertensive left ventricular hypertrophy and left heart failure.Methods A total of 35 patients with hypertensive left ventricular hypertrophy and left heart failure treated in NO.1 People′s Hospital of Jiujiang City in Jiangxi Province from March 2018 to March 2020 were selected as the study group,and 35 healthy subjects in the same period were selected as the control group.Both groups received echocardiography.The echocardiography diagnosis results of patients with different New York Heart Association(NYHA)grades in the study group,the control group,and the study group were observed.Results In the study group,the maximum diastolic blood flow velocity/diastolic annulus velocity(E/A)was(13.54±2.08),the left atrial diameter(LAD)was(31.51±4.36)mm,the left ventricular end-diastolic diameter(LVEDD)was(51.58±4.67)mm,and the left ventricular ejection fraction(LVEF)was(50.09±3.24)%.The E/EA and LVEDD of the study group were higher than the control group,and LAD and LVEF of the study group were lower than those of the control group(P<0.05).The E/EA value of the NYHA was(15.99±1.54),the LAD was(53.90±1.52)mm,the LVEDD was(59.87±5.34)mm,and the LVEF was(47.24±3.02)%.The comparison found that the E/EA value,LAD,and LVEDD of NYHA gradeⅢpatients in the study group were higher than those of NYHA gradeⅠandⅡpatients,and the LVEF was lower than NYHA gradeⅠandⅡpatients(P<0.05).Conclusion Cardiac ultrasound examination in the diagnosis of hypertensive left ventricular hypertrophy with left heart failure can judge the patient′s heart disease through the examination,and provide guidance for the diagnosis and the treatment plan for the next stage of the disease.
