摘  要:目的:探索心肺复苏时提高血钾浓度减轻大鼠脑组织脑缺血再灌注损伤的机制。方法:经食道电刺激诱导健康的SD大鼠心脏骤停(Cardiac Arrest,CA),按随机数据表法将CA大鼠随机分为生理盐水组(NS组,n=16)、氯化钾组(KCL组,n=16),CA 6 min时开始心肺复苏(Cardiopulmonary Resuscitation,CPR),并分别经静脉注射3.2 ml/kg生理盐水或2.5%氯化钾溶液。另随机选取12只大鼠作为假手术组(SH组,n=12),只进行麻醉及穿刺置管。记录恢复自主循环(Return of Spontaneous Circulation,ROSC)后30 min的平均动脉压,评估ROSC后24 h神经功能评分(NDS),检测大脑海马组织的活性氧(ROS)、丙二醛(MDA)水平和超氧化物歧化酶(SOD)活力。结果:与NS组对比,KCL组动物ROSC后平均动脉压无明显改变(P>0.05),ROSC后24hNDS明显升高,ROS、MDA水平降低(P<0.05)。结论:提高血钾浓度通过减轻氧化应激改善心肺复苏大鼠脑缺血再灌注损伤。Objective:To explore the mechanism by which the increase of serum potassium concentration during cardiopulmonary resuscitation(CPR)alleviates the cerebral ischemia-reperfusion injury in rats.Methods:Sprague-Dawley(SD)rats submitted to Cardiac Arrest(CA)by electrical induction were randomized into the normal saline group(NS group,n=16)and the potassium chloride group(KCL group,n=16)by random number table.Cardiopulmonary resuscitation initiated at 6 min after CA,simultaneously,were given 3.2 ml/kg normal saline or 2.5%potassium chloride solution via the left femoral vein,respectively.Another 12 rats were randomly selected as sham group(SH group,n=12),and only treated with anesthesia and puncture catheterization.The mean arterial pressure during 30 min after Return of Return of Spontaneous Circulation(ROSC)was recorded,and the neurological function score(NDS)was assessed at 24 h post ROSC.The levels of reactive oxygen species(ROS),malondialdehyde(MDA)and superoxide dismutase(SOD)of the hippocampal tissues were detected at 24 h post ROSC.Results:Compared with the NS group,the MAP of the KCL group had no significant change after ROSC(P>0.05),the NDS significantly increased and the levels of ROSand MDA significantly decreased at the 24 h post ROSC(P<0.05).Conclusion:Increasing serum potassium concentration during cardiopulmonary resuscitationalleviates cerebral ischemia-reperfusion injury via alleviating oxidative stress in rats.
