火针结合中药离子导入治疗类风湿性关节炎的临床研究

摘　　要：目的:研究火针结合中药离子导入治疗对类风湿性关节炎(RA)患者中医症状积分、炎症因子水平及关节功能的影响。方法:选择符合纳入标准的RA患者258例,依据随机数字表随机分为对照组及观察组,每组各129例。对照组给予西医常规治疗结合中药离子导入,观察组在对照组基础上给予火针治疗,两组患者均治疗2个月。检测两组患者治疗前后白细胞(WBC)、血小板计数(PLT)、血红蛋白(HB)、白细胞介素-1β(IL-1β)、白细胞介素-6(IL-6)及类风湿因子(RF)水平,并计算中医症状积分及关节功能指标(关节压痛指数、肿胀指数、晨僵时间指数)评分,统计不良反应发生率。结果:与治疗前比较,两组治疗后各项观察指标均改善明显(P<0.05);且治疗后观察组WBC、HB水平均高于对照组,PLT水平低于对照组(P<0.01),IL-1β、IL-6、RF水平均低于对照组(P<0.01),关节压痛指数、关节肿胀指数及晨僵时间指数均低于对照组(P<0.01),关于屈伸不利、拒按(压痛)和皮色发红的中医症状积分高于对照组(P<0.01);两组患者不良反应发生率比较,无统计学意义(P>0.05)。结论:火针结合中药离子导入治疗RA可明显改善患者临床症状和关节功能,并降低炎症因子水平。Objective:To study the effects of fire acupuncture combined with Chinese medicinal ion-introduction on scores of TCM symptoms,inflammatory factors indexes and joint function in the treatment of rheumatoid arthritis(RA).Methods:A total of 258 patients with RA were randomly divided into the control group and the observation group,with 129 patients in each group.The control group was given conventional western therapies combined with Chinese medicinal ion-introduction;on which basis,the observation group was also treated with fire acupuncture.Both groups were treated for 2 months.The levels of white blood cells(WBC),platelet count(PLT),hemoglobin(HB),interleukin-1β(IL-1β),interleukin-6(IL-6)and rheumatoid factor(RF)were detected,as well as the scores of TCM symptoms and joint function indexes(joint tenderness,joint swelling,morning stiffness time)were counted before and after the treatment in both groups.The incidence of adverse reactions was statistically analyzed.Results:Compared with those before the treatment,all the observation indexes were significantly improved after the treatment in the two groups(P<0.05).The levels of WBC and HB were higher and the level of PLT was lower in the observation group than those in the control group after the treatment(P<0.01);the levels of IL-1β,IL-6 and RF were lower in the observation group than those in the control group after the treatment(P<0.01);joint tenderness index,joint swelling index and morning stiffness time index were lower in the observation group than those in control group after the treatment(P<0.01);the scores of TCM symptoms,such as difficulties in flexion and extension,tender points and red skin,were higher in the observation group than those in the control group(P<0.01).There was no significant difference in incidence of adverse reactions between the two groups(P>0.05).Conclusion:Fire acupuncture combined with Chinese medicinal ion-introduction can improve TCM symptoms and joint function,and it can reduce the levels of inflammatory factors in the treatmen
