老中医治疗急性脑梗死的中医证型分析

摘　　要：目的通过数据挖掘探究首批国医大师治疗脑梗死急性期的证型分布规律。方法检索并整理国医大师经验书籍、公开发表的期刊文献,筛选首批国医大师治疗脑梗死急性期的医案数据,建立标准化医案数据库,采用统计分析、关联规则分析等方法,分析医案的证候出现规律。结果最终纳入93个医案。关联规则分析证候关联规则27条。其证候要素显示:风、血瘀、痰浊、火(热)、腑实、气虚、阴虚;脉象结果显示:弦脉、滑脉、数脉、沉脉、细脉;舌色结果显示:舌暗红频数最多,其次是舌红;苔质结果显示:苔腻最多,其次是苔白,苔厚,少苔,苔黄;最核心的证候组合是头晕+半身不遂+口舌歪斜,舌暗+苔白,苔腻+脉滑+舌暗。结论名老中医治疗急性脑梗死的病位主要涉及肝、脾、肾、心、胃,其常见的证候类型依次为痰浊血瘀、风火上扰、痰热腑实、气虚血瘀、阴虚风动,常见核心证型为痰浊血瘀。Objective To explore the type distribution rules of the first batch of Chinese medicine masters in treating acute cerebral infarction through data mining. Methods We search and sort out the experience books and published journals of national medical masters, and screen the medical case data of the first batch of national medical masters in the treatment of acute cerebral infarction,establish a standardized medical case database. Statistical analysis and association rule analysis are used to analyze the syndrome occurrence law of medical records. Results Finally, 93 medical cases are included. Association rules are used to analyse 27 association rules. Evidence elements show: wind, blood stasis, phlegm turbidity, fire(heat), visceral solid, deficiency, air deficiency, Yin deficiency;pulse results show: string pulse, smooth pulse, several pulse, deep pulse, fine pulse;the tongue color results show: the frequency of dark red tongue was the most, followed by red tongue;moss results show: the most moss, followed by white, thick, less moss and yellow. The core syndrome combination is dizziness + hemiplegia + crooked tongue, dark tongue + white fur, greasy fur + slippery pulse + dark tongue. Conclusion The location of acute cerebral infarction treated by famous old TCM doctors mainly involves liver, spleen, kidney, heart and stomach. The common syndrome types are phlegm turbidity and blood stasis, disturbance of wind and fire, phlegm heat and viscera excess, Qi deficiency and blood stasis, yin deficiency and wind movement, and the common core syndrome type is phlegm turbidity and blood stasis.
