摘 要:目的了解老年髋部骨折患者术前营养基本特征及营养不良的危险因素,有助于早期识别营养不良患者并尽早干预。方法收集2021年1月—6月期间就诊于北京积水潭医院创伤骨科急诊并拟接受髋部骨折手术的老年患者,进行营养评估,描述患者一般特征,采用Logistic回归分析老年髋部骨折患者术前营养不良的危险因素。结果共纳入448例患者,其中男性141例(31.4%),女性307例(68.6%),股骨颈骨折242例(54.0%),粗隆间骨折206例(46.0%)。合并脑卒中(OR=1.91,95%CI:1.23~4.31)、阿尔兹海默病(OR=21.92,95%CI:7.85~61.24)、慢性胃炎(OR=3.53,95%CI:1.34~9.30)、癌症(OR=7.41,95%CI:3.20~17.14)是老年髋部骨折患者术前发生营养不良风险的危险因素。结论对合并脑卒中、阿尔兹海默病、慢性胃炎、癌症的老年髋部骨折患者应尽早评估营养不良风险并干预。Objective To understand the risk factors of preoperative malnutrition in elderly patients with hip fracture and contribute to early identification of malnutrition patients and early intervention.Methods Collect the preoperative evaluation of elderly hip fracture patients in the emergency department from January to June 2021 to describe their general characteristics and analyze the risk factors for preoperative malnutrition in elderly patients with hip fracture.Results A total of 448 patients were included,including 141 males(31.4%),307 females(68.6%),242 transcervical fractures(54.0%),and 206 intertrochanter fractures(46.0%).Combined stroke(OR=1.91,95%CI:1.23~4.31),Alzheimer's disease(OR=21.92,95%CI:7.85 to 61.24),chronic gastritis(OR=3.53,95%CI:1.34 to 9.30),and cancer(OR=7.41,95%CI:3.20 to 17.14)are the risk factors for the risk of preoperative malnutrition in elderly patients with hip fracture.Conclusion The risk of malnutrition should be evaluated early in elderly hip fracture patients with stroke,Alzheimer's disease,chronic gastritis and cancer.
