益母草注射液应用于人工流产多中心真实世界研究

摘　　要：目的评价益母草注射液应用于人工流产女性的有效性和安全性,为临床合理用药提供参考。方法应用真实世界研究,采用前瞻性、非随机对照、多中心登记队列研究设计,共纳入2019年8月至2021年12月全国170家医院的人工流产女性8893例,其中观察组7506例,对照组1387例。对照组按其疾病诊治规范给予基础治疗,观察组在对照组基础上给予益母草注射液。最终观察组7473例、对照组1378例完成研究。观察两组女性术后阴道流血时间、术后阴道流血量、复经天数,并记录益母草注射液的不良反应发生情况。结果与对照组相比,益母草注射液应用于人工流产术,可有效缩短术后阴道流血时间(P<0.001),减少术后出血量(P<0.001),缩短复经时间(P<0.001);两组术后累计出血天数的构成情况(0~7d及7d以上)、术后累计出血量的构成情况(0~5mL、>5~<20mL、20~60mL、>60mL)差异有统计学意义(P<0.001)。结论益母草注射液应用于人工流产术,可有效缩短术后阴道流血时间,减少术后出血量,促进月经恢复,止血调经作用明显,值得临床推广应用。Objective To evaluate the efficacy and safety of motherwort injection in induced abortion patients,and to provide reference for clinical rational drug use.Methods This project was a real-world clinical study.It adopts a prospective,non-randomized controlled,multicenter registry cohort study design.A total of 8893 induced abortion patients from 170 hospitals across the country from August 2019 to December 2021 were included,including 7506subjects in the experimental group and 1387 subjects in the control group.Finally,7473 subjects in the experimental group and 1378 subjects in the control group completed the study.The patients in the control group were given basic treatment according to their disease diagnosis and treatment standards,and the experimental group was given motherwort injection on the basis of the treatment for the control group.The postoperative vaginal bleeding time,postoperative vaginal bleeding volume,and the interval from the completion of the induced abortion to the next menstrual cramp were observed in the two groups,and the adverse reactions of motherwort injection were recorded.Results Compared with the control group,motherwort injection applied to induced abortion could effectively shorten the postoperative vaginal bleeding time(P<0.001),reduce the postoperative bleeding volume(P<0.001),and shorten the interval from the completion of the induced abortion to the next menstrual cramp(P<0.001).The composition of postoperative cumulative bleeding days(0-7 days and more than 7 days)was statistically different between the two groups(P<0.001),and the composition of postoperative cumulative bleeding volume(0 to 5m L,5 to20m L,20 to 60m L,more than 60m L)had statistical difference(P<0.001).Conclusion Motherwort injection used in induced abortion can effectively shorten the postoperative vaginal bleeding time,reduce postoperative bleeding volume,promote menstrual recovery,and has obvious effects of hemostasis and menstruation regulation,which is worthy of clinical application.
