摘　　要：目的 基于纽曼系统模式,探讨城市养老机构老年专科护士服务现状,了解城市养老机构老年专科护士护理服务总体水平,分析护理服务开展中的制约因素,为推进城市养老机构老年专科护士护理服务开展提供依据。方法 采用系统抽样的方法,按照养老机构规模大小对某市养老机构进行排序、编号,在该市抽取14个养老机构,对120名老年专科护士进行问卷调查。调查内容包括一般人口学特征、开展程度、制约因素。采用描述性分析法研究城市养老机构老年专科护士护理服务总体水平、制约因素。结果 山东省某市城市养老机构老年专科护士护理服务项目总体开展情况得分为(65.55±8.41)分,处于一般水平。内部制约因素有老年专科护士人才匮乏、素质不高,老年专科护士的培养不规范、无标准,老年专科护士教育发展缓慢落后,老年专科护士岗位设置及职责定位模糊。外部制约因素有机构卫生资金不足,养老机构各项管理制度不完善,养老机构护理服务设施配置不全,机构护理服务内容与老人的需求不匹配,老年人养老观念落后。结论 提高养老机构护士护理服务水平应从多方面入手,政府应切实发挥引导作用;机构应提高护士待遇,增添医疗器械,改善护士工作,加强护士培训;国内高校应培养养老护理专业人才。Objective Based on the Newman system model,to explore the current status of elderly specialist nurses in urban elderly care institutions,to understand the overall level of nursing services for elderly specialist nurses in urban elderly care institutions,to analyze the constraints in the development of nursing services,and to promote the development of nursing services for elderly specialist nurses in urban elderly care institutions Provide evidence.Methods A systematic sampling method was adopted to sort and number the elderly care institutions in a city according to the size of the elderly care institutions.14 elderly care institutions were selected from the city,and a questionnaire survey was conducted among 120 specialized nurses for the elderly.The survey content includes general demographic characteristics,degree of development,and restrictive factors.The descriptive analysis method is used to study the overall level and restrictive factors of nursing services of elderly specialist nurses in urban elderly care institutions.Results The overall development of the nursing service project of the elderly specialist nurses in an urban elderly care institution in a city in Shandong Province scored(65.55±8.41)points,which was at an average level.Internal restrictive factors are lack of talents,low quality of senior nurses,irregular training of senior nurses,lack of standards,slow and backward development of senior nurses education,and fuzzy position setting and responsibilities of senior nurses.External constraints are insufficient funding for institutional health,imperfect management systems for elderly care institutions,incomplete configuration of nursing service facilities in elderly care institutions,mismatch of institutional care service content with the needs of the elderly,and failure to keep up with the concept of elderly care for the elderly.Conclusion The improvement of nursing care services of nursing care institutions in elderly care institutions should start from many aspects,and the government should
