摘 要:目的探究实时心肌声学造影(MCE)联合四维应变技术(4D-SI)在冠心病(CHD)诊断中的应用价值。方法选取高度疑似CHD患者80例为本研究组对象(最终确诊71例,按冠状动脉狭窄程度分为轻度27例,中度25例和重度19例),并选取同期健康体检志愿者30例为对照组,所有人员均接受MCE和4D-SI检查,评估诊断效能,并比较各组人员间心肌血供和心肌收缩期峰值应变情况。结果MCE联合4D-SI诊断灵敏度为91.55%,特异度为66.66%,准确度为88.75%,高于MCE联合自动成像技术检测78.87%、44.44%、75.00%,其中灵敏度、准确度比较差异具有统计学意义(P<0.05);与对照组比较,中度和重度狭窄长轴应变(4D-GLPS)、环向应变(4D-GCPS)、面积应变(4D-GAPS)和径向应变(4D-GRPS)均显著降低,轻度狭窄患者仅4D-GLPS和4D-GAPS显著降低(P<0.05)。结论MCE联合4D-SI可更加准确直观了解CHD患者心肌功能,辅助临床评估,诊断效能较高,其中4D-GLPS和4D-GAPS还可反映CHD早期病理改变,有利于其早期诊断。Objective To explore the application value of real-time myocardial contrast echocardiography(MCE)combined with four-dimensional strain technology(4D-SI)in the diagnosis of coronary heart disease(CHD).Methods 80 patients with highly suspected CHD admitted to our hospital were selected as the subjects of theresearch group(71 patients were finally diagnosed,and were divided into 27 mild cases,25 moderate cases and 19 severe cases).And 30 healthy volunteers during the same period were selected as the control group.All personnel underwent MCE and 4D-SI examinations to evaluate the diagnostic efficacy.Compare the myocardial blood supply and the peak strain of myocardial systole between the groups.Results The diagnostic sensitivity of MCE combined with 4D-SI was 91.55%,the specificity was 66.66%,and the accuracy was 88.75%,which was higher than that of MCE combined with automatic imaging technology(78.87%,44.44%,and 75.00%).Among them,the difference in sensitivity and accuracy was statistically significant(P<0.05).Compared with the control group,4D-GLPS,4D-GCPS,4D-GAPS and 4D-GRPS were significantly reduced.In patients with mild stenosis,only 4D-GLPS and 4D-GAPS were significantly reduced(P<0.05).Conclusion MCE combined with 4D-SI can more accurately and intuitively understand the myocardial function of CHD patients,assist clinical evaluation,and have a higher diagnostic efficiency.Among them,4D-GLPS and 4D-GAPS can also reflect the early pathological changes of CHD,which is conducive to its early diagnosis.
