摘 要:目的探讨基于OCTA图像分析对糖尿病视网膜病变患者黄斑区微血管病变观察.方法选择2021年1~10月收治的糖尿病视网膜病变患者37例(55眼)为试验组,再抽选同期健康体检者37例(60眼)为对照组,分析两组OCTA定量分析指标(VD_(1mm×1mm)、VD_(3mm×3mm)、VD_(1mm×3mm)、PD_(1mm×1mm)、PD_(3mm×3mm)、PD_(1mm×3mm)、黄斑中心凹无血管区域面积、周长与形态指数)情况,观察OCTA检测各指标对糖尿病视网膜病变的诊断优势.结果试验组在VD_(1mm×1mm)、VD_(3mmx3mm)、VD_(1mm×3mm)、PD_(3mm×3mm)、PD_(1mm×3mm)、黄斑中心凹无血管区域周长与形态指数均低(高)于对照组,互比,有差异性(P<0.05),但两组在PD_(1mm×1mm)与黄斑中心凹无血管区域面积指标互比,无显著差异(P>0.05).通过ROC曲线可得出,VD_(1mm×3mm)对糖尿病视网膜病变患者黄斑区微血管病变诊断价值较高,而黄斑中心凹无血管区域面积与周长对糖尿病视网膜病变患者黄斑区微血管病变诊断价值较小.结论OCTA图像分析可精确反映糖尿病视网膜病变患者黄斑区微血管病变状况,对尽早开展科学治疗对策、抑制病情发展等方面有着积极作用,且OCTA图像分析属于无创诊断,更易于患者接受与认可.Objective To study the macular microangiopathy in patients with diabetic retinopathy based on OCTA image analysis.Methods A total of 37 patients(55 eyes)of diabetic retinopathy patients were selected from January to October 2021 as the experimental group,and 37 healthy persons(60 eyes)in the same period were selected as the control group.The OCTA quantitative analysis index of the two groups was analyzed VD_(lmm×lmm),VD_(3mm×3mm),VD_(1mm×3mm),PD_(lmm×lmm),PD_(3mm×3mm),PD_(1mm×3mm),OCTA area,perimeter and morphology index of macular fovea,and observe the superiority of each index in diagnosing diabetic retinopathy.Results The experimental group was in Vd_(1mm×lmm),VD_(3mm×3mmm),VD_(lmm×3mm),PD_(3mm×3mm),PD_(lmm×3mm),the circumference and morphological index of the avascular area of the macular fovea were lower(higher)than those in the control group(P<0.05).There was no difference between PDlmmxlmin and the area index of avascular area in macular fovea(P>0.05).According to ROC curve,VDlmmx3mm had a high diagnostic value for microangiopathy in the macular area in patients with diabetic retinopathy,while the area and circumference of the macular fovea were less valuable for macular microangiopathy in patients with diabetic retinopathy.Conclusion OCTA image analysis can accurately reflect the condition of macular microangiopathy in patients with diabetic retinopathy,which has a positive impact on the early scientific treatment and the development of the disease.Moreover,it is a non-invasive diagnosis and is easier for patients to accept and recognize.
