摘 要:目的:探讨C反应蛋白(CRP)和乳酸脱氢酶(LDH)联合检测在评估重症肺炎患儿的病情及短期预后的临床作用。方法:将北京市顺义区妇幼保健院收治的70例重症肺炎患儿纳入研究,以小儿危重病例评分系统为参照依据将患儿分为非危重组(29例)、危重组(21例)、极危重组(20例),再根据患儿短期预后情况分为预后良好组(59例)和预后不良组(11例)。对患者外周血降钙素原(PCT)、白细胞计数(WBC)、中性粒细胞百分比(NEU)、CRP、LDH水平进行检测,利用受试者操作特征曲线(ROC)分析CRP、LDH对肺炎患儿病情病情及预后的评估作用。结果:70例肺炎患儿预后良好59例,占84.29%,预后不良11例,占15.71%。非危重组、危重组、极危重组之间PCT、WBC及NEU差异无统计学意义(P>0.05),非危重组、危重组、极危重组之间CRP、LDH水平差异有统计学意义(P<0.05);预后良好组患儿CRP、LDH水平低于预后不良组,差异有统计学意义(P<0.05);预后良好组患儿与预后不良组PCT、WBC及NEU差异无统计学意义(P>0.05)。采用Logistic回归分析显示CRP、LDH水平升高时导致肺炎患儿病情加重的不良因素(OR>1,P<0.05)。CRP和LDH联合检测ROC下面积高于CRP与LDH单独测定时ROC下面积。结论:肺炎患儿的病情与外周血CRP与LDH表达相关,CRP与LDH联合检测可作为临床评估肺炎短期预后的有效预测指标。Objective:To explore the clinical role of the combined detection of C-reactive protein(CRP)and lactate dehydrogenase(LDH)in evaluating the condition and short-term prognosis of children with severe pneumonia.Methods:70 children with severe pneumonia treated in a hospital were included in the study,and the children were divided into non-critical group(29 cases)and critical group(21 cases)and extreme ly critical group(20 cases)according to the pediatric critical case scoring system.According to the short-term prognosis,the children were divided into good prognosis group(59 cases)and poor prognosis group(11 cases).The levels of procalcitonin(PCT),leukocyte count(WBC),percentage of neutrophils(NEU),CRP and LDH in peripheral blood were detected,and the receiver operator characteristic curve(ROC)was used to analyze the evaluation effect of CRP and LDH on the condition and prognosis of children with pneumonia.Results:The prognosis of 70 children with pneumonia was good in 59 cases,accounting for 84.29%,and poor in 11 cases,accounting for 15.71%.There was no statistically significant difference in PCT,WBC and NEU among non-critical group,critical group and extremely critical group(P>0.05),and there was statistically significant difference in the levels of CRP and LDH among the three groups(P<0.05).The levels of CRP and LDH in good prognosis group were significantly lower than those in poor prognosis group,and the difference was statistically significant(P<0.05).There was no statistically significant difference in PCT,WBC and NEU between the good prognosis group and poor prognosis group(P>0.05).Logistic regression analysis showed that the increased levels of CRP and LDH were the adverse factors leading to the aggravation of pneumonia in children(OR>1,P<0.05).The area under ROC of combined detection of CRP and LDH was higher than that of CRP and LDH detection alone.Conclusion:The condition of children with pneumonia is related to the expression of CRP and LDH in peripheral blood.The combined detection of CRP and LDH can be
