改良腹膜前Kugel修补术治疗腹股沟疝的临床效果、并发症及对炎性指标的影响

摘　　要：目的:探讨采用改良腹膜前Kugel修补术对腹股沟疝进行治疗的临床效果及术后并发症发生情况,分析其对炎性指标所产生的影响。方法:选取2018年1月-2020年12月某院120例腹股沟疝患者,将其按照随机数字表法分为A组和B组各60例,A组采用腹腔镜经腹膜前疝修补术(TAPP),B组采用改良腹膜前Kugel修补术,对比两组各项手术指标以及术后发生并发症的情况,另比较两组手术前及术后24h时的炎性因子指标。结果:B组手术时间、住院时间均较A组短(P<0.05),术中出血量较A组少(P<0.05),术后并发症发生率较A组低(P<0.05)。B组术后24h时的肿瘤坏死因子-α(TNF-α)、超敏C反应蛋白(hs-CRP)、白细胞介素-6(IL-6)水平均较A组低(P<0.05)。结论:针对腹股沟疝患者,采用改良腹膜前Kugel修补术施治,效果好,并发症少,且能显著改善其炎症因子水平。Objective:To investigate the clinical effect and postoperative complications of modified preperitoneal Kugel repair for inguinal hernia,and analyze its influence on inflammatory indexes.Methods:Based on a specific period(January 2018-December 2020),120 patients with inguinal hernia in a hospital were selected and divided into two groups(60 cases in each group)according to the random number table method.Group A underwent laparoscopic transperitoneal hernia repair(TAPP),and group B underwent modified preperitoneal Kugel repair.The surgical indicators and postoperative complications were compared between the two groups.The inflammatory indexes before and at 24h after surgery were also compared between the two groups.Results:The operation time and hospital stay of group B were shorter than those of group A(P<0.05),the intraoperative blood loss was less than that of group A(P<0.05),and the incidence of postoperative complications was lower than that of group A(P<0.05).The levels of tumor necrosis factor-α(TNF-α),high-sensitivity C-reactive protein(hs-CRP),and interleukin-6(IL-6)in group B were lower than those in group A at 24h after operation(P<0.05).Conclusion:For patients with inguinal hernia,modified preperitoneal Kugel repair has good effect,fewer complications,and can significantly improve the levels of inflammatory factors.
