摘　　要：目的探讨不同髓内针直径(ND)与髓腔峡部直径(MCD)之比(ND/MCD)的弹性髓内针治疗股骨干骨折患儿的临床效果。方法回顾性分析医院2018年6月至2021年1月收治的90例股骨干骨折患儿的临床资料,全部患儿均接受弹性髓内针治疗,按照ND/MCD不同分为30%<ND/MCD<40%组(48例)与ND/MCD≥40%组(42例),比较两组围手术期指标、随访指标。结果30%<ND/MCD<40%组手术时间、住院时间、骨折愈合时间、取内固定时间均短于ND/MCD≥40%组,术中出血量少于ND/MCD≥40%组,Merchant膝关节评分高于ND/MCD≥40%组,差异均有统计学意义(P<0.05)。两组随访时间、畸形愈合率、骨折不愈合率、感染率、功能受限率比较,差异均无统计学意义(P>0.05)。结论在采用弹性髓内钉治疗股骨干骨折患儿时,选择30%<ND/MCD<40%的方案可获得更好的临床效果,且操作更加方便。Objective The clinical effects of elastic nails with different ratios of nail diameter(ND)to medullary canal diameter(MCD)(ND/MCD)in treating children with femoral shaft fractures were explored.Methods The clinical data of 90 children with femoral shaft fractures admitted to the hospital from June 2018 to January 2021 was retrospectively analyzed.All children were treated with elastic nail and were divided into a 30%<ND/MCD<40%group(48 cases)and a ND/MCD≥40%group(42 cases),and then the indexes of perioperative period and follow-up between the two groups were compared.Results The operation time,the hospital stay,the fracture healing time and the time for removing internal fixation in the 30%<ND/MCD<40%group were all shorter than those in the ND/MCD≥40%group,the intraoperative blood loss volume in the 30%<ND/MCD<40%group was less than that in the ND/MCD≥40%group,the Merchant's knee scores in the 30%<ND/MCD<40%group was higher than that in the ND/MCD≥40%group,with statistically significant differences(P<0.05).There were no statistically significant differences in the follow-up time,malunion rate,fracture nonunion rate,infection rate and functional limitation rate between the two groups(P>0.05).Conclusion It can obtain letter clinical effects to choose the scheme of ND/MCD>30%and<40%in treating children with femoral shaft fractures by elastic nails,and the operation is more convenient.
