冠状动脉斑块CT血管成像诊断结果与冠心病患者外周血小板功能的相关性分析

摘　　要：目的:探讨冠心病患者接受冠状动脉CT血管成像(computed tomography angiography,CTA)诊断斑块结果同外周血小板功能参数(血小板计数、平均血小板体积、血小板衍生生长因子含量、P选择素)间的关系。方法:选择2018年1月—2020年12月东莞市谢岗医院收治的60例完成CTA检查的冠心病患者作为观察组研究对象,选择同期于本院接受体检的健康者60名作为对照组,均接受外周血小板功能参数检查,同时观察组患者接受冠状动脉CT血管成像诊断,分析观察组患者检查结果及患者斑块类型情况,对比观察组不同斑块结果患者与对照组健康体检者外周血小板功能参数差异,探讨冠心病患者血小板功能参数同CT诊断结果间相关性。结果:观察组CTA诊断结果显示,11例未见斑块,13例为脂质斑块,14例为纤维斑块,12例患者为钙化斑块,10例患者为混合斑块。观察组无斑块患者血小板功能参数结果与对照组差异无统计学意义(P>0.05);观察组不同类型斑块患者血小板功能参数与对照组比较,差异均有统计学意义(P<0.05)。Pearson相关分析结果显示,冠心病患者CT值与血小板参数中PLC指标呈正相关,与P选择素、PDFG-BB及MPV呈负相关(P<0.05)。结论:通过冠状动脉CTA能够有效诊断冠心病患者斑块情况,不同斑块类型患者与血小板功能参数存在相关性,能够为临床疾病后续治疗提供可靠依据。Objective To explore the relationship between coronary artery CT angiography(CTA)diagnosis of plaque in patients with coronary heart disease and peripheral platelet function parameters(platelet count,average platelet volume,platelet-derived growth factor content,P-selectin).Methods From January 2018 to December 2020,60 patients with coronary heart disease who completed coronary artery CTA examinations were selected as the observation group research objects,and 60 healthy patients who received physical examinations in this hospital during the same period were selected as the control group research objects.The peripheral platelet function parameters were checked.At the same time,patients in the observation group received coronary artery CT angiography diagnosis.The examination results of patients in the observation group and the types of plaques were analyzed.The results of different plaques in the observation group were compared with those in the control group.Discuss the correlation between the parameters of platelet function and the results of CT diagnosis in patients with coronary heart disease.Result The CCTA diagnosis results of the observation group showed that 11 cases had no plaques,13 cases were lipid plaques,14 cases were fibrous plaques,12 cases were calcified plaques,and 10 cases were mixed plaques.The results of platelet function parameters of the observation group without plaques were not statistically different from those of the control group(P>0.05);the platelet function parameters of patients with different types of plaques in the observation group were compared with the control group,and the differences were statistically significant(P<0.05).The results of Pearson correlation analysis showed that the CT value of patients with coronary heart disease was positively correlated with PLC indicators in platelet parameters,and negatively correlated with P-selectin,PDFG-BB and MPV(P<0.05).Conclusion Coronary artery CTA can effectively diagnose plaque in patients with coronary heart disease.Patients with
