超声引导下射频消融术联合无水乙醇硬化治疗体表复杂静脉畸形的临床价值

摘　　要：目的:评价超声引导下射频消融技术联合无水乙醇硬化治疗体表复杂静脉畸形的临床效果。方法:选取2018年1月—2020年12月间临沂市肿瘤医院收治的76例体表复杂静脉畸形患者,随机分为试验组和对照组,各38例,两组均给予超声引导下无水乙醇硬化治疗,试验组辅以射频消融治疗,分析比较两组的治疗次数和不良反应发生情况,并对两组患者的近期疗效进行评价。结果:两组中治愈53例,有效23例,无效0例,治愈率69.7%,有效率100.0%。试验组治愈率86.8%(33/38)显著高于对照组的52.6%(20/38)(P<0.05)。试验组治疗(2.12±1.18)次,少于对照组的(3.24±1.22)次,差异有统计学意义(P<0.05)。另外,对照组无水乙醇注射后出现2例面神经损伤,1例腓总神经损伤,经对症处理后恢复,试验组无神经损伤症状者。结论:超声引导下射频消融术联合无水乙醇硬化治疗体表复杂静脉畸形具有很好的疗效和安全性,应用前景广阔。Objective To investigate the clinical effect of Ultrasound-guided radiofrequency ablation combined with ethanol injection sclerotherapy in treating complicated venous malformation.Methods 76 patients with complex venous malformations on the body surface treated in Linyi tumor hospital from January 2018 to December 2020 were randomly divided into experimental group and control group,with 38 cases in each group,both groups were treated with absolute ethanol injection sclerotherapy under ultrasound guidance,and the experimental group was supplemented with radiofrequency ablation.The treatment times and adverse reactions of the two groups were analyzed and compared,and the short-term curative effects of the two groups were evaluated.Results In the two groups,53 cases were cured,23 cases were effective and 0 cases were ineffective.The cure rate was 69.7%and the effective rate was 100.0%.The cure of the experimentation group 86.8%(33/38)was significantly higher than those of the control group 52.6%(20/38)(P<0.05).The treatment times(2.12±1.18)was less than that of the control group(3.24±1.22),the difference was statistically significant(P<0.05).In addition,2 cases of facial nerve injury and 1 case of common peroneal nerve injury occurred after absolute ethanol injection in the control group,which recovered after symptomatic treatment.There were no symptoms of nerve injury in the experimental group.Conclusion Ultrasound-guided RFA technology combined with absolute ethanol injection sclerotherapy in treating complicated venous malformation has the exact efficacy and high safety,the clinical application value is relatively high.It is suitable for clinical promotion and application.
