摘　　要：目的观察首诊恶性肿瘤血液病患者在应用医院-家庭(H2H)规范化营养管理模式下,临床营养状况的改善及应用疗效。方法选择2018年1月—2019年12月医院肿瘤内科以及血液内科首次确诊为恶性肿瘤血液病患者100例为研究对象,按照组间基本特征均衡可比的原则分为观察组和对照组,各50例。对照组实施常规营养管理,观察组实施“H2H”营养护理管理模式。观察两组干预1,6个月时人体学测量指标、实验室检查、营养状况自评分(PG-SGA)及癌症疲乏评分。结果干预1个月,两组患者人体学测量指标、实验室检查、PG-SGA及癌症疲乏评分比较差异均无统计学意义(P>0.05)。干预6个月,两组患者人体学测量指标各项数值均优于对照组(P<0.05),观察组实验室指标白蛋白、血红蛋白及血清前白蛋白较对照组相比均明显提高(P<0.05),观察组患者化疗后PG-SGA、癌性疲乏评分低于对照组(P<0.05)。结论“H2H”规范化营养管理模式在首诊恶性肿瘤血液病患者中能够更好地改善患者的营养状态,降低癌性疲乏发生率,提高生活质量,增加首诊患者的治疗依从性,更加便于医患沟通。Objective To observe the improvement of clinical nutritional status and the effi cacy of the application of the hospital to home(H2H)standardized nutrition management model in the fi rst-diagnosed malignant hematological patients.Methods A total of 100 patients with hematological diseases diagnosed with malignant tumor for the fi rst time in the medical oncology department and hematology department of the hospital from January 2018 to December 2019 were selected as the study subjects.According to the principle of balance and comparability of basic characteristics between the groups,they were divided into an observation group and a control group,with 50 patients in each group.The control group was given routine nutritional management,and the observation group was given the"H2H"nutritional care management mode.Anthropometric measurements,laboratory tests,nutritional status self-rating(PG-SGA)and cancer fatigue score were observed in the two groups at 1 and 6 months of intervention.Results After 1 months of intervention,there was no signifi cant diff erence in the anthropometric measurement indicator,laboratory tests,PG-SGA and cancer fatigue score between the two groups(P>0.05).After 6 months of intervention,the anthropometric measurement indexes of the two groups were better than those of the control group(P<0.05).Compared with the control group,the laboratory indexes of albumin,hemoglobin and serum prealbumin in the observation group were signifi cantly improved(P<0.05).The scores of PG-SGA and cancer fatigue were lower than those of the control group(P<0.05).Conclusion The application of"H2H"standardized nutrition management model can better improve the nutritional status of patients with hematological diseases of malignant tumor,reduce the incidence of cancer fatigue,improve the quality of life,increase the treatment compliance of fi rst-diagnosed patients,and be more convenient for medical treatment.suff er from communication.
