“笑脸”形胶布导管外固定法在PICC导管护理中的效果评价

摘　　要：目的观察“笑脸”形胶布导管外固定法在神经外科PICC护理中的应用效果。方法按照组间基本资料均衡可比的原则将2020年1—5月PICC置管的50例患者设为对照组,2020年6—12月PICC置管的50例患者设为观察组。对照组采取传统固定法,使用静脉置管换药包,按标准流程常规消毒、3M透明敷料固定外露导管及连接器,再使用免缝胶带蝶形交叉外固定;观察组按照标准流程消毒处理后,在透明敷料与连接器处使用“笑脸”形胶布进行导管固定。比较两组患者导管移位及脱出率、维护耗时和间隔天数、导管留置时间及满意程度等。结果对照组发生22例导管移位,观察组发生3例导管移位,观察组少于对照组(P<0.05);两组未发生导管脱出。对照组单次PICC维护平均耗时24.07±1.53 min,观察组平均耗时22.59±1.74 min,观察组维护耗时短于对照组(P<0.05)。对照组PICC维护间隔时间为3.92±2.45 d,观察组间隔时间为5.60±2.18 d,观察组维护间隔时间长于对照组(P<0.05)。观察组患者的满意程度优于对照组(P<0.05)。结论“笑脸”形胶布导管固定方法能有效固定PICC导管,可有效避免导管移位,减少PICC维护时间,提高护理质量。Objective To observe the application eff ect of"smiley face"-shaped tape catheter external fi xation in neurosurgery PICC nursing.Methods According to the principle of balance and comparability of basic data between groups,50 patients with PICC catheterization from January to May 2020 were set as a control group,and 50 patients with PICC catheterization from June to December 2020 were set as an observation group.In the control group,the traditional fi xation method was adopted,the IV catheter was used to change the dressing kit,the routine disinfection was performed according to the standard procedure,the 3M transparent dressing was used to fi x the exposed catheter and the connector,and then the butterfl y cross external fi xation was used without seam tape;After the observation group was sterilized according to the standard procedure,the catheter was fi xed with a"smiley face"-shaped tape at the transparent dressing and the connector.The catheter displacement and prolapse rate,maintenance time and interval days,catheter indwelling time and satisfaction degree were compared between the two groups.Results There were 22 cases of catheter displacement in the control group,3 cases in the observation group,and the observation group was less than the control group(P<0.05);no catheter prolapse occurred in the two groups.The average maintenance time of a single PICC in the control group was 24.07±1.53 minutes,the average time in the observation group was 22.59±1.74 minutes,and the maintenance time in the observation group was shorter than that in the control group(P<0.05).The maintenance interval of PICC in the control group was 3.92±2.45 days,while that in the observation group was 5.60±2.18 days,and the maintenance interval in the observation group was longer than that in the control group(P<0.05).The satisfaction of patients in the observation group was better than that in the control group(P<0.05).Conclusion The"smiley face"-shaped tape catheter fi xation method can eff ectively fi x the PICC catheter,can eff ec
