集束化护理对结肠镜检查患者肠道准备、舒适度和满意度的影响

摘　　要：目的分析集束化护理对结肠镜检查患者肠道准备、舒适度和满意度的影响。方法选取2018年1月至2020年1月于解放军总医院第六医学中心进行结肠镜检查的92例门诊患者为研究对象,按照数字表法随机分为对照组和试验组,每组46例,对照组患者予常规护理,试验组患者在常规护理的基础上予集束化护理。对比2组患者的生命体征指标(呼吸频率、心率、平均动脉压)、应激反应(肾上腺素、去甲肾上腺素、皮质醇)水平、Boston肠道准备评分、状态焦虑量表(STAI‐S)评分、特质焦虑量表(STAI‐T)评分、舒适度、满意度及不良反应发生情况。结果检查中15 min、检查中30 min、检查后试验组患者的呼吸频率、平均动脉压均小于对照组(P<0.05)。检查后,试验组患者血清肾上腺素、去甲肾上腺素、皮质醇水平均低于对照组(P<0.05);试验组患者的Boston肠道准备评分高于对照组(P<0.05);试验组患者的STAI‐S评分和STAI‐T评分低于对照组(P<0.05);试验组患者的舒适度和满意度明显高于对照组(P<0.05);试验组患者的不良反应发生率明显低于对照组(P<0.05)。结论集束化护理应用于结肠镜检查,能够稳定患者的生命体征指标,减轻患者的应激反应,使肠道准备更加充分,同时还能够缓解患者的焦虑情绪,提高结肠镜检查的舒适度和患者的满意度,且更加安全,值得临床推广。Objective To analyze the impact of clustered care on bowel preparation,rates of comfort and satisfaction before colonoscopy.Methods 92 outpatients who underwent colonoscopy at the Sixth Medical Center of General Hospital,from January 2018 to January 2020 were selected as research objects.They were divided into the control group and the experimental group,in accordance with the random number table method,each consisting of 46 patients.The patients in the control group were given routine nursing,while those in the experimental group received cluster nursing on the basis of routine nursing.Comparisons were made in such vital signs as respiration,heart rate,mean arterial pressure,stress response(epinephrine,norepinephrine,cortisol),Boston bowel preparation scores,state anxiety scale(STAI‐S)scores,trait anxiety(STAI‐T)scores,rates of comfort and satisfaction,as well as adverse reactions of the two groups.Results The respiration and mean arterial pressure of the experimental group 15 minutes during examination,30 minutes during examination,and after the examination,were lower than those of the control group(P<0.05).After examination,the serum epinephrine,norepinephrine,and cortisol levels of the examination group were lower than those of the control group(P<0.05).The Boston bowel preparation scores of the examination group were higher than those of the control group(P<0.05).The STAI‐S scores and STAI‐T scores of the test group were lower than those of the control group(P<0.05).The rates ofcomfort and satisfaction of the examination group were significantly higher than those of the control group(P<0.05).The rate of adverse re‐actions of the examination group was significantly lower than that of the control group(P<0.05).Conclusion The application of cluster nursing before colonoscopy could stabilize patient’s hemodynamics,relieve patient’s stress response,and make the bowel better prepared for colonoscopy.At the same time,it could reduce patient’s anxiety and improve the rates of comfort and satisfaction
