关节镜下腘窝囊肿单向活瓣切除的临床疗效分析

摘　　要：目的研究关节镜下腘窝囊肿单向活瓣切除术这种手术方式治疗腘窝囊肿的临床中期疗效。方法选择2018年4月—2020年4月在右江民族医学院附属医院就诊的48例腘窝囊肿患者。其中男性20例,女性28例,年龄49~75岁,平均(60.50±6.50)岁。术前Rauschning-Lindgren分级Ⅱ级35例,Ⅲ级13例,采用关节镜下腘窝囊肿单向活瓣切除术治疗,记录患者的手术时长、术后关节积液量和并发症、住院周期、VAS评分、术后Rauschning-Lindgren分级及术后复发情况。结果手术时长(46.80±4.60)min、住院周期(5.60±1.00)d、手术后VAS评分[术后(2.40±1.60)分vs术前(7.50±2.20)分]和术后Rauschning-Lindgren分级(0级39例、Ⅰ级9例)相比术前(Ⅱ级35例、Ⅲ级13例)明显改善,差异具有统计学意义(P<0.05)。结论采用关节镜下腘窝囊肿单向活瓣切除术治疗腘窝囊肿康复快、并发症少以及获得满意的临床效果,值得进一步推广。Objective To study the mid-term clinical efficacy of arthroscopic unilateral valvectomy for popliteal cyst.Methods This study selected a total of 48 patients with popliteal cyst treated in The Affiliated Hospital of Youjiang Medical University for Nationalities from April 2018 to April 2020.There were 20 males and 28 females,aged from 49 to 75 years,with an average age of(60.50±6.50)years.The preoperative Rauschning-Lindgren grading showed 35 cases of gradeⅡand 13 cases of GradeⅢ.They were treated with arthroscopic unilateral valvectomy for popliteal cyst.It was necessary to record the duration of surgery,postoperative joint effusion and complications,duration of hospitalization,VAS scores,postoperative Rausching-Lindgren grades and postoperative recurrence.Results The duration of surgery was(46.80±4.60)min.The duration of hospitalization was(5.60±1.00)d.The postoperative VAS score was(2.40±1.60)in contrast with the preoperative VAS scoreas(7.50±2.20).In comparison with the grading before surgery,postoperative Rauschning-Lindgren grading(39 cases of grade 0,9 cases of gradeⅠ,35 cases of gradeⅡ,13 cases of gradeⅢ)were significantly improved,and the difference was statistically significant(P<0.05).Conclusion Arthroscopic unilateral valvectomy for popliteal cyst has the advantage of rapid recovery with few complications and satisfactory clinical results.Therefore,it is worthy of further promotion.
