摘　　要：目的:探讨静脉注射用丙种球蛋白(IVIG)冲击疗法治疗免疫性血小板减少症(ITP)患儿的临床效果及对T淋巴细胞的影响。方法:选取治疗的ITP患儿130例为研究对象,按治疗方案的不同分为对照组(n=64)与对照组(n=66),对照组给予泼尼松治疗,观察组在对照组基础上给予IVIG冲击疗法,观察两组治疗疗效。结果:观察组治疗总有效率优于对照组(Z=-2.884,P=0.004);观察组治疗后血小板为(120.01±8.29)×10^(9)/L,明显高于对照组(t=14.929,P=0.000);观察组治疗后Th细胞为(40.02±6.60)%,明显高于对照组(t=3.130,P=0.002);观察组和对照组治疗后Ts细胞比较差异无统计学意义(t=-1.194,P=0.235);观察组和对照组不良反应发生率比较差异无统计学意义(χ^(2)=0.144,P=0.704)。结论:IVIG冲击疗法治疗ITP患儿有较好的效果,能明显提高患儿血小板、Th细胞水平。Objective To explore the clinical effect of intravenous immunoglobulin(IVIG)shock therapy on children with immune thrombocytopenia(ITP)and its effect on T lymphocyte.Method 130 children with ITP treated in our hospital were selected,They were divided into two groups according to different treatment schemes,control group(n=64)was treated with prednisone,the observation group(n=66)received IVIG shock therapy on the basis of the control group,the therapeutic effect of the two groups were observed.Results The therapeutic effect of the observation group was better(Z=-2.884,P=0.004),and the total effective rate was 95.45%;The platelet in the observation group after treatment was(120.01±8.29)×10^(9)/L,which was significantly higher than that in the control group(t=14.929,P=0.000);After treatment,Th cells in the observation group were(40.02±6.60)%,which was significantly higher than those in the control group(t=3.130,P=0.002);There was no significant difference in Ts cells between the observation group and the control group after treatment(t=-1.194,P=0.235).The difference in incidence of adverse reactions between the observation group and the control group was not statistical significance(χ^(2)=0.144,P=0.704).Conclusion IVIG shock therapy has a good effect in treating ITP children,and can significantly improve the level of platelet and Th cells in children.
