摘 要:目的探讨绝经后妇女肌肉、脂肪与骨密度之间的相关性,为绝经妇女肌少-骨质疏松症(OP)、肌少性肥胖的预防提供一定的参考。方法回顾性分析2019年1月—2020年1月本院就诊的152例绝经后妇女的临床资料,根据是否为OP分为OP组(74例)和对照组(非OP,78例),收集所有受检者年龄、身高、体重和体质量指数信息。采用双能X线骨密度仪测量受检者腰椎(L_(1)-L_(4))、髋部骨密度及肌肉、脂肪含量,计算相对四肢骨骼肌质量指数(RASMI)。结果OP组与对照组在年龄、身高、体质量、体质量指数方面相比较,差异无统计学意义(P>0.05)。OP组全身肌肉量、躯干肌肉量、双上肢肌肉量及双下肢肌肉量低于对照组,差异均有统计学意义(P<0.05)。OP组全身脂肪量、体脂率、躯干脂肪量及双下肢脂肪量均高于对照组,差异均有统计学意义(P<0.05)。OP组肌肉不足比例为94.59%,对照组肌肉不足比例为29.49%,两组肌肉不足情况比较,差异有统计学意义(P<0.05)。OP组体脂率增高比例为70.27%,对照组体脂率增高比例为43.59%,两组体脂率增高比较,差异有统计学意义(P<0.05)。全身肌肉量、躯干肌肉量及双下肢肌肉量与腰部、髋部骨密度分别呈正相关,差异均有统计学意义(P<0.05)。体脂率、双下肢脂肪量与腰部、髋部骨密度分别呈负相关,差异均有统计学意义(P<0.05)。腰椎、髋部骨密度与肌肉、脂肪量多元线性回归分析结果表明,腰椎骨密度、髋部骨密度的变化与躯干肌肉量、双下肢肌肉量及体脂率有多元线性回归关系,差异均有统计学意义(P<0.05)。结论绝经后骨质疏松妇女中存在肌肉减少、脂肪含量增加的情况,临床上及时检测肌肉、脂肪含量,可及早识别、及早预防肌少-骨质疏松症的发生。Objective To understand the correlation between muscle,fat and bone mineral density in postmenopausal women,so as to provide some basis for the prevention of myopenia osteoporosis(OP)and myopenia obesity in postmenopausal women.Methods Retrospective analysis method was employed in this study.152 postmenopausal women those were ever been admitted to our hospital during January 2019 and January 2020 were enrolled as study objects.According to the diagnostic criteria of osteoporosis,they were divided into osteoporosis group(74 cases)and control group(non OP,78 cases).The age,height,weight and body mass index of all subjects were collected.The bone mineral density of lumbar spine(L_(1)-L_(4))and hip were measured by dual energy X-ray bone density instrument,muscle and fat content were measured as well.The relative limb skeletal muscle mass index(RASMI)was calculated.Results There were no significant differences in age,height,weight and body mass index between OP group and control group(P>0.05).The total body muscle mass,trunk muscle mass,double upper limb muscle mass and double lower limb muscle mass in OP group were lower than those in control group(P<0.05).The total body fat,body fat rate,trunk fat and lower limb fat in OP group were higher than those in control group(P<0.05).The proportion of muscle deficiency in OP group was 94.59%and that in control group was 29.49%,there was significant difference between the two groups(P<0.05).The increased proportion of body fat rate in OP group was 70.27%,and that in control group was 43.59%,there was significant difference between the two groups(P<0.05).The whole body muscle mass,trunk muscle mass and lower limb muscle mass were positively correlated with the bone mineral density of waist and hip respectively(P<0.05).The body fat rate and lower limb fat mass were negatively correlated with the bone mineral density of waist and hip respectively(P<0.05).The results of multiple linear regression analysis showed that the changes of lumbar bone mineral density and hip bone miner
