经阴道二维和三维超声检查对宫角附近异位妊娠的临床诊断价值

摘　　要：目的探讨经阴道二维和三维超声检查对间质部、峡部、子宫角部异位妊娠的临床诊断价值。方法选取在山东中医药大学附属医院生殖与遗传中心就诊患者,以宫腔镜或者腹腔镜证实宫角附近异位妊娠,并术前均进行过二维及三维超声检查者为观察对象,回顾分析其超声检查结果,探讨不同方式超声检查对宫角附近异位妊娠的临床诊断价值。结果73例宫角附近异位妊娠,腹腔镜检查证实输卵管间质部妊娠16例,二维超声检查误诊3例,三维超声检查误诊2例;腹腔镜检查证实输卵管峡部妊娠29例,二维超声检查误诊9例,三维超声检查误诊4例;宫腔镜检查证实宫角妊娠28例,二维及三维超声检查全部正确诊断。结论三维超声对宫角附近异位妊娠的诊断准确率高于二维超声检查。Objective To investigate the clinical value of transvaginal two-dimensional ultrasound and three-dimensional ultrasound in the diagnosis of ectopic pregnancy in uterine horn,stroma and isthmus.Methods Patients were selected from the Reproductive and Genetic Center of Integtated Traditional and Western Medicine Affiliated Hospital of TCM University.The patients with ectopic pregnancy near uterine horn confirmed by endoscopy were examined by two-dimensional and three-dimensional ultrasonography before operation.We reviewed the results of two-dimensional and three-dimensional ultrasonography before invasive examination,and explored the clinical diagnostic value of different ways of ultrasonography.Results There were 73 cases of ectopic pregnancy near the uterine horn,Laparoscopy confirmed 16 cases of tubal interstitial pregnancy,3 cases were misdiagnosed by two-dimensional ultrasound and 2 cases were misdiagnosed by three-dimensional ultrasound.29 cases of tubal isthmus pregnancy were confirmed by laparoscopy.9 cases were misdiagnosed by two-dimensional ultrasound,and 4 cases were misdiagnosed by three-dimensional ultrasound.28 cases of cornual pregnancy confirmed by hysteroscopy,all cornual pregnancy were correctly diagnosed by two-dimensional and three-dimensional ultrasonography.Conclusion The accuracy of three-dimensional ultrasound in diagnosing ectopic pregnancy near cornu uteri is higher than that of two-dimensional ultrasound.
