达格列净联合胰岛素泵强化治疗对糖尿病患者胰岛β细胞功能及氧化应激的影响

摘　　要：目的探究在糖尿病患者中采用达格列净+胰岛素泵强化治疗的临床效果。方法选择2019年6月—2021年9月该院收治的糖尿病患者108例,按随机数表法对照组和观察组,各54例。对照组予以二甲双胍+胰岛素泵强化治疗(2周后停用),观察组加用达格列净。对比两组胰岛β细胞功能、氧化应激反应及不良反应。结果治疗后,观察组空腹胰岛素(FINS)(11.35±2.39)mU/L、胰岛β细胞功能(HOMA-β)(5.90±1.22)、谷胱甘肽过氧化物酶(GSH-PX)(78.91±8.39)U/L、超氧化物歧化酶(SOD)(131.17±10.42)U/mL水平均高于对照组,差异有统计学意义(t=4.784、4.082、12.639、16.145,P<0.001),丙二醛(MDA)(8.09±1.54)nmol/mL低于对照组,差异有统计学意义(t=7.679,P<0.001);两组不良反应发生情况比较,差异无统计学意义(χ^(2)=0.441,P=0.507)。结论在糖尿病患者中采用达格列净+胰岛素泵强化治疗能够改善胰岛β细胞功能,减轻氧化应激反应,且安全性高。Objective To explore the clinical effect of intensive treatment with dapagliflozin+insulin pump in diabetic patients.Methods A total of 108 diabetic patients admitted to the hospital from June 2019 to September 2021 were selected,according to the random number table method,the control group and the observation group,54 cases in each.The control group was given intensive treatment with metformin+insulin pump(discontinued after 2 weeks),and the observation group was additionally given dapagliflozin.The isletβ-cell function,oxidative stress and adverse reactions were compared between the two groups.Results After treatment,fasting insulin(FINS)(11.35±2.39)mU/L,islet beta cell function(HOMA-β)(5.90±1.22),glutathione peroxidase(GSH-PX)(78.91±8.39)U/L,superoxide dismutase(SOD)(131.17±10.42)U/mL levels,were higher than the control group,and the difference was statistically significant(t=4.784,4.082,12.639,16.145,P<0.001).Malondialdehyde(MDA)(8.09±1.54)nmol/mL was lower than the control group,and the difference was statistically significant(t=7.679,P<0.001).There was no statistically significant difference in the incidence of adverse reactions between the two groups(χ^(2)=0.441,P=0.507).Conclusion Intensive therapy with dapagliflozin+insulin pump in diabetic patients can improve isletβ-cell function and reduce oxidative stress with high safety.
