加速康复外科在钬激光前列腺剜除围手术期护理中的应用

摘　　要：目的:探讨在钬激光前列腺剜除手术中围手术期快速恢复外科理念(Enhanced recovery after surgery,ERAS)在护理中的应用体会。方法:将76例前列腺钬激光剜除手术(holmium laser enucleation of the prostate,HoLEP)患者随机分为实验组和对照组各38例,实验组采取加速康复外科理念进行护理,而对照组采用传统围术期护理方式,比较两组患者围手术期护理效果。结果:两组患者住院费用、冲洗时间、术后住院时间、手术时间、尿路感染、继发出血、深静脉血栓、发热、肺部感染比较差异有统计学意义(P<0.05),而术后尿失禁以及尿潴留的发生无统计学意义(P>0.05)。结论:应用加速康复外科理念可缩短HoLEP术患者术后住院时间,降低住院费用,减少术后尿管留置时间及并发症。Objective:To investigate the application of Enhanced recovery after surgery(ERAS)in perioperative nursing of holmium laser enucleation of prostate.Methods:Seventy-six patients with holmium laser enucleation of the prostate(HoLEP)were selected and randomly divided into the experimental group and the control group,38 patients in each group.The experimental group adopted the concept of accelerated rehabilitation surgery for nursing,and the control group adopted the traditional perioperative nursing.The perioperative nursing effect was compared between the two groups.Results:There were statistically significant differences in hospitalization cost,irrigation time,postoperative hospital stay,operation time,urinary tract infection,secondary bleeding,deep vein thrombosis,fever and pulmonary infection between the two groups(P<0.05).There was no significant difference in the incidence of postoperative urinary incontinence and urinary retention(P>0.05).Conclusion:The application of accelerated rehabilitation surgical nursing concept can shorten the postoperative hospitalization time of HoLEP patients,reduce the hospitalization cost,and reduce the postoperative catheter indwelling time and complications.
