摘 要:目的探讨多岗协作血糖强化管理在糖尿病患者冠状动脉旁路移植术(CABG)术后血糖管理中的应用效果。方法本研究选取2019年2月—2020年2月河南省某医院收治的98例行CABG的糖尿病患者作为研究对象。采用随机数字表法将患者分为对照组和研究组,每组49例。对照组患者采用常规血糖管理,研究组患者在此基础上行多岗协作血糖强化管理。比较2组患者干预前及干预后(出院时)空腹血糖(FBG)、餐后血糖(PBG)及糖化血红蛋白(Hb Alc)水平。比较2组患者出院时并发症发生情况及呼吸机使用时间、伤口愈合时间、住院时间。结果干预前,2组患者FBG、PBG、Hb A1c水平比较,差异无统计学意义(P>0.05)。干预后,2组患者FBG、PBG、Hb A1c水平均低于干预前,且研究组低于对照组,差异均有统计学意义(P<0.05)。研究组患者并发症发生率为6.12%,低于对照组的32.64%,差异有统计学意义(P<0.05)。研究组患者呼吸机使用时间、伤口愈合时间、住院时间均短于对照组,差异均有统计学意义(P<0.05)。结论基于多岗协作血糖强化管理可降低糖尿病患者CABG术后FBG、PBG、Hb A1c水平,降低并发症发生率,缩短呼吸机使用时间、伤口愈合时间及住院时间,临床效果显著,值得临床推广应用。Objective To analyze the use of multi-post collaboration-based intensive blood glucose management in diabetic patients underwent coronary artery bypass grafting(CABG).Methods A total of 98 diabetic patients admitted to a hospital for CABG in Henan Province from February 2019 to February 2020 were selected as subjects,who were assigned to a control group and an experimental group using the random number method,with 49 cases in each group.The patients in the control group received routine blood glucose management,and the patients in the experimental group multi-post collaboration-based intensive blood glucose management in addition to the routine management.The fasting blood glucose(FPG),postprandial blood glucose(PBG)and hemoglobin A1c(Hb Alc)levels were compared before and after(at discharge)intervention between the two groups.The incidence of complications,ventilator use time,wound healing time,and length of hospital stay were compared between the two groups.Results Before the intervention,there was no statistically significant difference in FBG,PBG,and Hb A1c levels between the 2 groups(P>0.05).After the intervention,the levels of FBG,PBG and Hb A1c were lower in both groups than before the intervention,and the experimental group were lower than those in the control group,with statistically significant differences(P<0.05).The incidence of complications in the experimental group was 6.12%,which was lower than that in the control group(32.64%),and the difference was statistically significant(P<0.05).The ventilator use time,days for wound healing,and length of hospital stay of patients in the experimental group were shorter than those in the control group,and the differences were statistically significant(P<0.05).Conclusion The multi-post collaboration-based intensive management of blood glucose can reduce FBG,PBG,Hb A1c levels in diabetic patients underwent CABG,decrease the incidence of complications,reduce ventilator use time,days for wound healing,and length of hospital stay,with makred clinical effects.These
