精索静脉曲张患者精液质量下降的危险因素及预后分析

摘　　要：目的探讨精索静脉曲张(VC)患者精液质量下降危险因素及手术预后。方法取2020年1月至2021年1月收治入郑州大学第一附属医院的215例VC患者为研究对象,根据其精液水平及前向运动精子百分率分为正常组和异常组,分析与精液质量下降相关的危险因素,其中手术治疗3个月后复查精液常规的患者91例,分析与精液质量提升相关的预后因素。结果精液水平与卵泡刺激素(FSH)呈负相关(r=-0.308,P=0.007),年龄较大的患者更容易出现精液水平异常(OR=2.820,P=0.029),术前精液水平低的患者更容易改善(OR=0.082,P=0.003),术前精液前向运动精子百分率低的患者更容易改善(OR=0.110,P=0.002)。结论FSH可作为预测精液水平异常的良好指标,截断值为3.49 IU·L^(-1),术前精液质量较差的患者可以获得更多的改善。Objective To investigate the risk factors of varicocele(VC)semen quality decline and the prognosis of surgery.Methods A total of 215 VC patients admitted to the First Affiliated Hospital of Zhengzhou University from January 2020 to January 2021 were selected as the research objects.According to their semen concentration and the proportion of forward sperm movement,they were divided into normal group and abnormal group,and the risk factors related to the decline of semen quality were analyzed.Among them,91 patients underwent routine semen reexamination after 3 months surgical treatment,and the prognostic factors related to the improvement of semen quality were analyzed.Results Semen concentration was negatively correlated with follicle stimulating hormone(FSH)(r=-0.308,P=0.007).Older patients were more likely to have abnormal semen concentration(OR=2.820,P=0.029).Patients with a low preoperative semen concentration were more likely to improve(OR=0.082,P=0.003),and patients with a low preoperative semen activity ratio were more likely to improve(OR=0.110,P=0.002).Conclusion FSH can be used as a good indicator for predicting abnormal semen concentration.The cut-off value is 3.49 IU·L^(-1),and patients with poor preoperative semen quality can get more improvement.
