摘　　要：目的探究药品集中带量采购政策实施前后医院用药结构、运行绩效及患者费用负担相关指标的变化,为推动政策实施提供参考。方法比较分析某三级甲等公立综合医院用药结构、运行绩效及患者费用负担在政策实施前后的变化情况。结果政策实施1年后,医院中标药品使用率均大幅提高,药品需求结构从以心血管系统药物为主转变为以抗肿瘤药物为主,60%的中标药品及同通用名药品用药频度呈上升趋势,使用金额构成比变化明显等;药占比和国家基本药物配备使用金额比例降低,门诊/住院费者次均药品费用增幅下降明显;中标药品及同通用名药品限定日费用均呈下降趋势。结论药品集中带量采购政策显著改变了某院的用药结构,降低了医院药占比,有利于降低患者用药负担。Objective To explore the changes in a hospital’s drug use structure,performance,and patient’s cost before and after the implementation of the centralized drug procurement policy,thus informing the policy implementation.Methods To compare and analyze the changes in the drug structure,operation performance and patient cost burden of a tertiary grade A public hospital before and after the implementation of the policy.Results One year after the implementation of the policy,the rate of bid-winning drugs was greatly increased.The demand structure for drugs changed from mainly cardiovascular drugs to anti-tumor drugs.About 60%of the bid-winning drugs and drugs with the same generic name are on an upward trend,and the composition of the amount used has changed significantly;the proportion of drug cost and the proportion of the amount used in the national basic drug equipment has decreased,and the increase in the average drug cost per outpatient/inpatient rate has decreased significantly.All the defined daily dose consumption for the winning drug and the approved drug showed a downward trend.Conclusion The centralized drug procurement policy has significantly changed the drug use in studied hospital,reduced the proportion of drugs cost,and decreased patients’cost of drug use.
