摘　　要：目的探讨慢性心力衰竭患者血清利钠肽活化酶(Corin)、心钠素水平与病情严重程度及预后的关系。方法回顾性选取2020年1月至2021年7月在中南大学湘雅医学院附属海口医院治疗的慢性心力衰竭患者116例,作为观察组,同时选取100名健康志愿者作为对照组。比较两组血清Corin、心钠素水平,同时分析观察组不同心功能分级、预后患者血清Corin、心钠素水平。分析观察组患者Corin与心钠素相关性;分析Corin、心钠素预测慢性心力衰竭预后的价值。结果观察组血清Corin为(445.52±94.43)ng/L,明显低于对照组[(614.45±101.20)ng/L],而心钠素为(75.50±14.49)pg/mL,明显高于对照组[(22.40±9.91)pg/mL],差异均有统计学意义(P<0.05)。观察组NYHA分级Ⅳ级患者Corin为(377.53±90.02)ng/L,明显低于Ⅱ级和Ⅲ级患者[(512.43±89.20)ng/L、(433.65±81.15)ng/L],心钠素为(93.25±13.22)pg/mL,明显高于Ⅱ级和Ⅲ级患者[(57.70±11.20)pg/mL、(78.82±14.41)pg/mL]。观察组预后良好患者Corin为(463.05±90.01)ng/L,明显高于预后不良患者[(417.47±88.82)ng/L],而心钠素为(68.81±14.42)pg/mL,明显低于预后不良患者[(86.06±13.31)pg/mL](P<0.05)。Corin与心钠素呈负相关(r=-0.412,P<0.05)。Corin、心钠素预测慢性心力衰竭患者预后不良的ROC曲线下面积分别为0.717和0.788。结论慢性心力衰竭患者Corin水平降低,而心钠素水平升高,Corin、心钠素水平与病情程度及预后相关,且在预测患者预后方面有一定价值。Objective To investigate the relationship between the levels of serum natriuretic peptide activating enzyme(Corin)and atrial natriuretic peptide(ANP)and the severity and prognosis of chronic heart failure.Methods A total of 116 patients with chronic heart failure treated in Haikou Affiliated Hospital of Central South University Xiangya School of Medicine from January 2020 to July 2021 were retrospectively selected as the observation group,and 100 healthy volunteers were selected as the control group,the levels of serum Corin and ANP between the two groups were compared,and the levels of serum Corin and ANP in patients with different functional grades and prognosis in the observation group were analyzed.The correlation between Corin and atrial natriuretic peptide in the observation group was analyzed;the value of Corin and atrial natriuretic peptide in predicting the prognosis of chronic heart failure was analyzed.Results The level of Corin in the observation group was(445.52±94.43)ng/L,which was significantly lower than that in the control group[(614.45±101.20)ng/L],while the level of ANP was(75.50±14.49)pg/mL,which was significantly higher than that in the control group[(22.40±9.91)pg/mL],the differences were statistically significant(P<0.05).The level of Corin of patients with NYHA gradeⅣin observation group was(377.53±90.02)ng/L,which was significantly lower than that of patients with gradeⅡandⅢ[(512.43±89.20)ng/L,(433.65±81.15)ng/L],the level of ANP was(93.25±13.22)pg/mL,which was significantly higher than that of patients with gradesⅡandⅢ[(57.70±11.20)pg/mL,(78.82±14.41)pg/mL],the differences were statistically significant(P<0.05).In the observation group,the level of Corin of patients with good prognosis was(463.05±90.01)ng/L,which was significantly higher than that of patients with poor prognosis[(417.47±88.82)ng/L],while the ANP was(68.81±14.42)pg/mL,which was significantly lower than that of patients with poor prognosis[(86.06±13.31)pg/mL],the differences were statistically signif
