探析25-羟基维生素D3与2型糖尿病肾病患者尿Netrin-1、Angptl4水平及肾功能的相关性

摘　　要：目的探讨25-羟基维生素D_(3)与2型糖尿病肾病患者尿神经导向因子-1(Netrin-1)和血管生成素样蛋白4(Angptl4)及肾功能指标的相关性。方法回顾性选取2018年1月至2020年1月石家庄市第二医院收治的2型糖尿病肾病患者103例作为研究对象,根据尿清蛋白/肌酐比值及血肌酐值将其分成A、B、C组(即微量蛋白尿组、大量蛋白尿组和肾功能不全组),各27、59、17例;同时选61名健康志愿者作对照(D组)。采用酶联免疫吸附试验检测25-羟基维生素D_(3)及尿Netrin-1、Angptl4水平,而肾功能指标则采用全自动生化分析仪测定;用Pearson分析25-羟基维生素D_(3)与尿Netrin-1、Angptl4及肾功能指标的相关性。结果A组的25-羟基维生素D_(3)、尿Netrin-1、Angptl4表达水平及估算肾小球滤过率(eGFR)均高于B、C组,低于D组,血肌酐水平均低于B、C组,高于D组,差异均有统计学意义(P<0.05)。相关性结果发现,25-羟基维生素D_(3)与尿Netrin-1、Angptl4、eGFR呈正相关(r=0.108、0.432、0.132,P<0.05),25-羟基维生素D_(3)与血肌酐呈负相关(r=-0.152,P<0.05),与Ca、P无相关性(P>0.05)。结论2型糖尿病肾病的25-羟基维生素D_(3)与其尿Netrin-1、Angptl4表达及eGFR呈正相关,与血肌酐呈负相关,在一定程度上可反映2型糖尿病肾病的严重程度。Objective To investigate the correlation between 25-hydroxyvitamin D_(3) and urinary nerve guide factor-1(urine Netrin-1),angiopoietin-like protein 4(Angptl4)and renal function indexes in patients with type 2 diabetic nephropathy.Methods A total of 103 patients with type 2 diabetic nephropathy admitted to the Second Hospital of Shijiazhuang City from January 2018 to January 2020 were retrospectively selected as the research subjects.According to the ratio of urinary albumin to creatinine and the value of serum creatinine,they were divided into groups a,B and C(i.e.microalbuminuria group,massive albuminuria group and renal insufficiency group),27,59 and 17 cases each,and 61 healthy volunteers were selected as control group(group D),25-hydroxyvitamin The expression levels of D_(3),Netrin-1 and Angptl4 in urine were detected by enzyme-linked immunosorbent assay,while the renal function indexes were measured by automatic biochemical analyzer.The correlation of 25-hydroxyvitamin D_(3) with urinary Netrin-1,Angptl4 and renal function indexes was analyzed by Pearson.Results The expression levels of 25-hydroxyvitamin D_(3),urinary Netrin-1,Angptl4 and estimated glomerular filtration ratee(eGFR)in group A were higher than those in group B and C,but lower than those in group D,and Scr level was lower than those in group B and C,but higher than that in group D,the differences were statistically significant(P<0.05).The correlation results found that 25-hydroxyvitamin D_(3) was positively correlated with urinary Netrin-1,Angptl4 and eGFR(r=0.108,0.432,0.132,P<0.05),while 25-hydroxyvitamin D_(3) was negatively correlated with Scr(r=-0.152,P<0.05),no correlation with Ca and P(P>0.05).Conclusion In type 2 diabetic nephropathy,25-hydroxyvitamin D_(3) was positively correlated with urinary netrin-1,ANGPTL4 expression and eGFR,and negatively correlated with Scr,which could reflect the severity of type 2 diabetic nephropathy to some extent.
